The Role

The Strategic Partnerships Liaison is the growth engine for Abby Care within the community case management ecosystem. Your mission is to build, own, and scale referral-generating relationships with case management professionals and organizations. Unlike traditional sales, this role requires a deeply consultative approach. You aren't just pitching a service; you are educating partners, solving their placement challenges, and becoming their absolute go-to resource for patient care.

Your goal is to spread awareness, build immense trust, and drive a sustained, high volume of referrals to Abby Care. Because this is a fundamental growth position, your performance targets and compensation structure are directly linked to the referral volume and successful admissions generated from your territory.

Your Responsibilities

  • Build & Own Relationships: Manage the entire lifecycle of partner relationships in your territory, from initial cold outreach and discovery to ongoing account management and referral troubleshooting.

  • Execute Market-Specific Strategies:

    • Navigate Payer organizations to connect directly with embedded Payer Case Managers and their leadership, bypassing broad corporate structures. Additionally, build strong partnerships with Support Coordinators and Support Coordination Organizations (SCOs) under the ODP Program.

  • Consultative Education: Teach case managers exactly who fits our programs, how to initiate a referral effortlessly, and why Abby Care delivers the best outcomes for the families they manage.

  • Drive Targeted Referral Volume: Meet and exceed ambitious referral quotas. You will be compensated based on the success and volume of the relationships you build.

  • Field-Based Activity: Maintain a highly active field schedule. Prioritize face-to-face meetings, community networking, and regular check-ins over office time.

  • Pipeline Management: Treat your territory like a business. Rigorously track all partner interactions, account statuses, and referral conversion metrics in our CRM.

What you’ll have:

  • 4+ years of experience navigating Payer organizations, managed care environments, or ODP/waiver-funded services. With a proven ability to build relationships inside complex, layered organizations rather than just at the surface level.

  • An understanding of Pennsylvania's Payer and ODP ecosystem. Experience working with or alongside Payer Case Managers, Support Coordinators and Support Coordination Organizations is a strong advantage.

  • The skill to bypass bureaucracy and reach decision makers. This market rewards people who know how to navigate organizations strategically and build trust at every level.

  • Territory ownership with real stakes. You'll drive referral volume in a market that requires relationship depth, not just breadth.
