What You’ll Do

Provide mobile primary care visits to seniors
Manage chronic and acute conditions
Perform comprehensive assessments & care planning
Collaborate with caregivers, facilities, and interdisciplinary teams
Deliver compassionate, patient-centered care
Maintain accurate EMR documentation

Qualifications

Active NP or PA license
1+ years of primary care, geriatrics, home health, hospice, SNF, or senior care experience preferred
Strong communication and clinical skills
Valid driver’s license & reliable transportation
