Duties:

- Coordinate and oversee the MDS (Minimum Data Set) process for patients in a healthcare setting
- Ensure accurate and timely completion of MDS assessments and documentation
- Collaborate with interdisciplinary team members to gather necessary information for MDS assessments
- Review medical records and interview patients, families, and caregivers to gather relevant information
- Analyze data and identify potential issues or discrepancies in MDS assessments
- Develop care plans based on MDS assessments and collaborate with healthcare professionals to implement appropriate interventions
- Monitor and evaluate patient progress and update care plans as needed
- Provide education and training to staff on MDS documentation requirements and processes

Skills:

- Proficiency in using point click care or similar electronic health record (EHR) systems
- Experience working with children, including knowledge of child development and age-appropriate care practices
- Knowledge of infection control protocols and practices to ensure patient safety
- Familiarity with family planning services and resources
- Knowledge of discharge planning processes and resources for smooth transitions of care
