Job Description

In this role you will:

  • Support NDIS participants, along with their families and carers, to identify their needs through assessment and person-centred planning
  • Owning the assessment, development and delivery of evidence-based support plans within a complex support environment
  • Driving meaningful goals and measurable outcomes, using a positive behaviour support framework and a commitment to eliminating restrictive practices
  • Ensuring plans reflect each participant’s goals, strengths and what matters most to them
  • Building the capability, confidence and understanding of those supporting participants in their day-to-day lives
  • Working collaboratively within a multidisciplinary team, including mentoring less experienced practitioners

Qualifications

You will need:

  • NDIS Registration as a Behaviour Support Practitioner 
  • Tertiary qualifications in a relevant allied health discipline or a related area 
  • Current driver's license and own reliable vehicle
  • A genuine passion for working in the disability sector
