Ennoble Care is seeking a full-time Patient Care Coordinator to join our team in an office-based setting in the Jackson, Mississippi area. This role is an integral, in-office member of our clinical care team, working alongside fellow Care Coordinators and collaborating with Ennoble Care providers within a family-practice-style model of care.
While not patient-facing, the position plays a critical role in supporting patient-centered care coordination, documentation, and communication, helping ensure continuity of care and timely follow-up across care settings.

KEY RESPONSIBILITIES
- Coordinate and manage patient care activities through Ennoble Care’s electronic medical record (EMR) system, supporting continuity of care and positive patient outcomes
- Support day-to-day clinical workflows by assisting with referrals, laboratory orders and follow-up, medication refill requests, durable medical equipment (DME) coordination, and other care coordination tasks
- Maintain frequent communication with patients, providers, families, and interdisciplinary teams to address patient needs, resolve barriers to care, and ensure timely follow-up
- Document all patient interactions accurately and communicate changes in condition, concerns, or urgent matters promptly to the appropriate clinical team members
- Demonstrate professionalism, empathy, strong organizational skills, and excellent customer service while effectively managing multiple priorities in a fast-paced environment
- Utilize EMR systems, secure email, Microsoft Office, telephone systems, and other office technology while contributing positively to daily operations and team collaboration
QUALIFICATIONS
- Legally authorized to work in the United States of America
- Minimum of 1 year of experience in a clinical and administrative or care coordination role within a healthcare setting
- Highly preferred: Experience supporting providers, coordinating patient care, and/or working in a medical office/family practice setting
- Comfortable spending a significant portion of the day on the phone communicating with patients/families and care teams
- Demonstrates professionalism, compassion, empathy, and exceptional customer service with a patient-centered approach
- Strong organizational, documentation, and time-management skills with the ability to multitask and manage multiple priorities in a fast-paced environment
- Professional communication skills across phone, email, Microsoft Teams, and in-person interactions
- Proficient in Microsoft Office applications, including Outlook, Word, and Excel, and comfortable learning new technologies and EMR systems
- Ability to adapt positively to change, work collaboratively as part of a team, and continuously learn new skills
- Must be able to work full-time, Monday through Friday, 8:30am to 5:00pm CST, on-site/in-office in Jackson, MS
- No weekends, evenings, nights, or on-call requirements
- Seven (7) company-paid holidays annually
- Must successfully complete a background check and drug screen
COMPENSATION & BENEFITS
- Starting hourly: $17-$18/hr
- Full-time benefits eligibility after 30 days of employment
