At Cheer Home Care, we are looking for a dedicated Registered Nurse (RN) who loves spending quality, one-on-one time with older adults and delivering premium, high-quality care right in the comfort of their homes. In this concierge role, you will collaborate closely with an interdisciplinary team to develop, implement, and manage individualized care plans while providing continuous support and feedback to clients, their families, and your nursing peers.

Key Responsibilities & Duties

• Comprehensive Assessments: Conduct thorough clinical assessments of clients' physical, emotional, and environmental needs.
• Care Planning & Goal Setting: Develop customized, evidence-based care plans and establish measurable health and wellness goals for each client.
• Chronic Care Management: Oversee the ongoing management of chronic conditions, ensuring proactive and preventative interventions.
• Post-Surgical & Wound Care: Provide specialized skilled nursing interventions, including post-operative care, wound care, and medication management as needed.
• Client & Family Education: Serve as a trusted resource by educating clients and their representatives on diagnoses, treatment plans, and self-care strategies.
• Holistic Wellness Support: Deliver comprehensive wellness support that promotes independence, safety, and an enhanced quality of life for senior

Requirements

• Current, unrestricted California registered nurse license

• 3 years of experience as a registered nurse
• Home Health experience preferred
• Proof of Negative TB result
• Physical Screening Documentation within the last year
• Active California Drivers license and car insurance
• Cell phone with Internet Access

Hourly Pay Rate: $48-$75 based on experience. Potential for team bonus.
