Private Duty Nurse (RN/LPN) – 1:1 Home Care
Location: Clio/Flint/Saginaw/Lake Orion
Setting: In-home, one-on-one care
Job Summary
Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting.
Patient #1: Pediatric Care
- Location: Flint, MI
- Patient: 10-year old boy
- Day Shift: 2:00pm-2:00am
Patient #2: Pediatric Care
- Location: Saginaw, MI
- Patient: 1-Year-Old Female
- Day Shift: 7:00 AM – 7:00 PM (Sun–Sat)
- Night Shift: 7:00 PM – 7:00 AM (Sun–Sat)
Patient #3: Adult Care
- Location: Lake Orion, MI
- Patient: 49-Year-Old Female (Paraplegic)
- Schedule: 24/7 Skilled Nursing Coverage (12-Hour Shifts)
- Day Shift: 7:00 AM – 7:00 PM
- Night Shift: 7:00 PM – 7:00 AM
Patient #4: Adult Care
- Location: Clio, MI
- Patient: (56-Year-Old Male)
- Shifts: 6 AM – 4 PM (Tue & Thu)
Responsibilities
- Provide skilled nursing care (trach/vent care, medication administration, catheter care)
- Monitor and document patient condition and vital signs
- Assist with daily living activities (ADLs)
- Communicate with families and care teams
- Maintain safe, high-quality, patient-centered care
Schedule & Pay
- Full-time, part-time, and per diem shifts available
- Day, night, and weekend shifts
- Weekly pay + optional daily pay

Qualifications
- Active RN or LPN license in the state of Michigan
- Home care or private duty experience preferred (not required)
- Trach/vent experience a plus (training provided)
- Strong communication and critical thinking skills
Patient Population
- Pediatrics (birth–21 years)
- Adults with complex medical needs
