Advanced Practitioner – Independent Futures

Our client, based in Staffordshire, is seeking an experienced Advanced Practitioner to join their Independent Futures Team within Adult Social Care.
This is a key front-door leadership role within a high-demand, fast-paced environment, where you will provide oversight, risk assessment and decision-making on referrals received with often limited information.
In addition to managing complex safeguarding and assessment activity, you will play a crucial role in supporting and supervising a mixed workforce of qualified and unqualified staff, ensuring a timely, proportionate and high-quality front door service.
Key Responsibilities
Core duties include:
- Providing professional oversight and triage of incoming referrals, ensuring appropriate thresholds and pathways are applied
- Undertaking robust risk assessments based on limited or emerging information, ensuring defensible and proportionate decision-making
- Leading on complex cases and escalations, offering guidance and direction to the wider team
- Delivering effective supervision and support to both qualified and unqualified staff, promoting best practice and continuous development
- Ensuring a timely, responsive front door assessment service, balancing demand and risk effectively
- Working collaboratively with partner agencies to ensure co-ordinated and person-centred responses
- Supporting the team to apply relevant legislation, including safeguarding and mental capacity frameworks
- Promoting strengths-based practice and outcome-focused interventions
- Ensuring high standards of record-keeping, decision logs and reporting, in line with statutory and organisational requirements
- Contributing to service development and continuous improvement within the Independent Futures Team
- Applying professional curiosity and critical thinking to ensure risks are fully explored and managed
Qualifications & Experience
Professional Qualification:
- MA, BA, BSc in Social Work, Diploma in Social Work or CQSW
Registration:
- Must be registered with Social Work England
Experience:
- Minimum 2+ years’ post-qualified experience (including ASYE completion)
- Significant experience within Adult Social Care front-door/triage services
- Experience managing complex referrals and high-risk cases
- Experience providing supervision, mentoring or practice leadership to staff
- Strong background in risk assessment and decision-making under pressure
- Experience working within a fast-paced, high-demand service environment
Legislation Knowledge:
Strong working knowledge of:
- Care Act 2014
- Mental Capacity Act 2005 (MCA)
- Human Rights Act 1998 (HRA)
- Safeguarding Adults procedures and frameworks
Skills:
- Advanced assessment and analytical skills
- Strong leadership and supervisory capability
- Ability to make sound, defensible decisions in complex situations
- Confidence working in a pressurised, high-volume environment
- Excellent communication and partnership-working skills
- Ability to prioritise and manage competing demands effectively
- High standard of written documentation and reporting
- Demonstrates professional curiosity, resilience and sound judgement
Other Requirements:
- Full UK driving licence and access to a car is desirable
