Title: Case Manager
Job Duties: The Case Manager is responsible for assisting in planning, developing, organizing, implementing, and evaluating the Activity Program and Special Treatment Program (STP) in accordance with current existing federal, state, and local standards, as well as established policies and procedures. The Case Manager is responsible for case management in order to ensure that residents are given ample and regular opportunities to learn and practice skills for improving the level of functioning, independence, awareness, self-care, and self-esteem.

Qualifications:
- Bachelor's degree required; major in psychology, social work or behavioral sciences preferred.
- Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
- Familiarity with regulatory standards.
- Experience in mental health preferred.

About the Campus: Crestwood Manor in Fremont is a skilled nursing facility serving individuals from a number of California communities. The facility provides long-term treatment services to people with a primary medical diagnosis and specialized behavioral management needs – for example, Alzheimer’s or dementia.
