Attentive Care is a CHAP-accredited, family- and veteran-owned home health care and staffing agency dedicated to providing compassionate in-home care and facility support services across the Capital Region. As a Licensed Practical Nurse (LPN), you will play a vital role in delivering high-quality patient care within facility settings, supporting both residents and staff, and ensuring the well-being of those entrusted to your care.
Key responsibilities include:
- Providing direct patient care in facility environments, including assisted living, rehab facilities, nursing homes, and adult day care
- Administering medications and treatments according to provider orders
- Monitoring, assessing, and documenting vital signs and patient status
- Collaborating with facility staff and promptly reporting changes in patient condition
- Supporting staffing relief coverage across assigned shifts and multiple facilities

Requirements
Required Qualifications:
- Current New York State Licensed Practical Nurse (LPN) license
- Recent experience working as an LPN in a facility setting (such as assisted living, rehab facility, nursing home, or adult day care)
- Experience administering medications and treatments per provider orders in a facility setting
- Experience monitoring, assessing, and documenting vital signs and patient status in a facility setting
- Experience collaborating with facility staff and reporting patient condition changes
- English proficiency for patient and staff communication
- Authorized to work in the United States
- Reliable transportation and ability to travel up to 25 miles between assigned facilities
Preferred Qualifications:
- Experience with facility EMR/documentation/charting systems
- BLS/CPR certification
