Job Overview 

We are looking for a Nurse Practitioner to work with patients to help them manage addiction and comorbid health conditions. Duties include meeting with patients one on one, via telehealth to provide medication management services in a medical practice that serves patients with a variety of addictive disorders.   

The ideal candidate will be a responsible and well-trained professional able to give the best care with little supervision. Providers must be compassionate, detail-oriented, and adaptable to EBP models of care.   

The goal is to promote patient well-being by providing the highest quality of interdisciplinary care over a longer term, which improves clinical outcomes.  

This position will report directly to the CEO, Amanda Wilson, MD. 

Primary job duties and responsibilities: 

  • Perform evaluations of patients’ needs and assist in the development of customized treatment plans. 
  • Provide individuals with the resources they need to reach personal goals. 
  • Coordinate with social and medical agencies, external medical and psychiatric healthcare providers and community outreach programs to provide patients with needed services. 
  • Complete continuing education courses to maintain licensure and certification and stay up to date on the latest practices.  
  • Treat social/psychological and behavioral disorders in collaboration with an interdisciplinary team. 
  • Provide direct treatment management as well as supervision, evaluation and prescribing of psychotropic medications. 
  • Provide after-hours consultation/contact for urgent client situations in the caseload. 
  • Attends required staff and team meetings and case conferences, for the purpose of providing professional input into executing a plan of treatment and assessment of other pertinent issues. 
  • Participates in medical team/program staff meetings and business meetings as scheduled. 
  • Prescribes psychotropic medications and conducts regular assessments of therapeutic responses and side effects.  
  • Educates patients regarding psychotropic medications and side effects.  
  • Works collaboratively as a team member within all disciplines.  
  • Performs other medical duties as requested within the scope of the position. 
  • Expands knowledge and capabilities by attending educational workshops, conferences etc. 
  • Participate in interdisciplinary meetings to provide the highest quality patient care. 
  • Collaborate with physicians, other APCs, and nurses to develop individualized care plans for patients. 
  • Foster a supportive and compassionate environment to care for patients and their families. 
  • Performing other duties as assigned by the CEO. 
  • Collaborate with navigators, therapists, and peer guides, or other treatment team members to coordinate patient care. 
  • Responsible for timely and accurate completion of medical records, including assessment, treatment plan, progress notes, and outcome measures in our electronic medical record. 
  • Examine or order laboratory or diagnostic tests for clients to provide information on general physical condition and mental disorders. 
  • Return calls and emails promptly (within 24 hours of receipt or sooner if clinically necessary). 
  • Provides crisis assessment and management of patients and accessible to NorthStar Care on-call providers.  
  • Responsible for making independent decisions based on protocols developed utilizing national guidelines for treatment interventions. 
  • Follow all procedures and protocols as per NorthStar Care policy, federal and state regulations.  
  • Actively participate in all departmental education, training, and in-services to confirm understanding of new information or changes to established clinical policies and procedures. 

Requirements

Job Requirements and Skills: 

  • MINIMUM REQUIREMENT – Must possess appropriate credentials of professional qualifications and current licensure in at least one state as a Nurse Practitioner. Must have an NPI number or be able to obtain one upon employment. 
  • EXPERIENCE – addiction experience preferred.  
  • PRODUCTIVITY TOOLS - Solid knowledge of key productivity tools like Microsoft O365 (PowerPoint, Excel, Word, etc.), or equivalent. 
  • VIDEO CONFERENCING – Familiarity with administering, optimizing, and/or licensing video conference tools like Microsoft Teams, Zoom, GoToMeeting, Webex, or equivalent. 
  • MESSAGING PLATFORMS – Familiarity with administering, optimizing, and/or licensing message platforms like Microsoft Teams, Webex Teams, Slack, or equivalent. 
  • ELECTRONIC MEDICAL RECORDS (EMR) – Familiarity with electronic medical records (EMR) systems and documentation requirements to assist clinicians, nurses, navigators, and peer guides with workflow and documentation best practices. 
  • ADDITIONAL
