Schedule: Part-time flexibility (pick up visits based on your availability; weekdays and/or weekends as needed)
Location: Field Based
Compensation (Productivity Points): $75.00 / point

The mission of this role
Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.
Objectives
Carry a high-quality caseload
Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.
What you'll own
Complete in-home OT evaluations and deliver skilled OT visits.
Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
Recommend and train on adaptive equipment and home modifications as needed.
Coordinate with nursing and the broader care team when condition or needs change.
Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.

Must-haves
Active Michigan OT license (in good standing)
Strong clinical judgment and comfort working independently in the field
Comfort escalating changes in condition to RN/MD and documenting rationale clearly
Strong communication and documentation
Reliable follow-through on orders, frequencies, and patient goals
Valid driver’s license, auto insurance, and reliable transportation and comfort with daily travel across assigned territory.
Able to carry/transport basic therapy equipment as needed
Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)
Nice-to-haves
Prior home health experience
Homecare Homebase (HCHB) experience (or similar home health EMR)
Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
Strong organization and time management
