Job Title: Home Health Licensed Vocational Nurse (LVN) Job Type: Full-time Location: South Bay, CA

Support when you need it. Autonomy when you want it.

At Health Link Home Health & Hospice, we believe that providing great patient care starts with supporting our clinicians. For nearly 20 years, we have served the Bay Area with a reputation for excellence—not just in how we treat our patients, but in how we treat our team.

We are currently seeking a compassionate Licensed Vocational Nurse (LVN) for our South Bay territory. In this role, you will work independently in the field while staying connected to a dedicated Director of Nursing and back-office support team who handle the logistics so you can focus on the patient.

Why LVNs Choose Health Link

  • Real-Time Clinical Support: You are never alone in the field. Our back office is staffed by experienced clinicians ready to help if you have a complex patient situation, need to call in a lab, or are running late.

  • Low-Pressure Scheduling: Flexible schedules with realistic production goals. We don’t want you rushing; we want you providing quality care.

  • True Overtime Pay: Unlike many roles, our LVNs are non-exempt. If you work late, take an extra shift, or handle an extra visit, you are paid fairly in accordance with California Overtime Guidelines.

  • Quality Over Quantity: Bonuses are based on quality metrics, not just how many patients you squeeze into a day.

  • Modern Tools: Company-issued iPad and all necessary work tools provided.

Compensation & Benefits

  • Salary Range: $83,200 - $104,000 per year (Based on $40.00 - $50.00/hour + Overtime eligibility)

  • Health: Medical, Dental, Vision, and Life Insurance

  • Retirement: 401(k) with employer matching

  • Perks: Paid Time Off (accrues for actual use, up to 10 days), Vehicle Reimbursement (or company vehicle option), and Referral Program.

Your Role

  • Implement the plan of care initiated by the Registered Nurse.

  • Provide skilled nursing care including wound care and medication administration.

  • Educate patients and family members on diagnosis and care plans.

  • Collaborate with the interdisciplinary team (RN, PT, OT, ST, MSW) and report identified needs to the Clinical Supervisor.

  • Complete accurate and timely documentation in Kinnser on company-provided devices.

  • Maintain effective communication with the back-office team to ensure seamless care coordination.

Requirements

  • Current California Licensed Vocational Nurse (LVN) license.

  • IV Certification and wound care/wound nursing experience required.

  • Minimum one year of previous nursing experience in the medical field.

  • Valid California Driver's License.

  • Home Health experience is recommended but not required

Who This Role Is For

This role is for an LVN who wants the freedom of home health without the feeling of being unsupported. If you are organized, skilled in wound care, and looking for a stable agency that respects your time and pays for your overtime.
