Home Health Scheduling Coordinator (Full-Time)
Location: Walnut Creek, CA (On-site)
Pay: $27–$32 per hour DOE
Experience: 1 year Home Health experience preferred

Health Link Home Health and Hospice is a leading provider of comprehensive home health services throughout the San Francisco Bay Area and East Bay for over two decades. We are currently seeking a self-driven, detail-oriented Scheduling Intake Coordinator to join our Walnut Creek team.

Key Responsibilities

Maintain and coordinate schedules for home health services
-
Assist with intake and process incoming referral information
-
Ensure cases are assigned and staffed within established timeframes
-
Communicate schedule updates to clinicians, patients, supervisors, and referral sources

Manage high call volumes with professionalism and a pleasant demeanor
-
Respond to urgent or emergency scheduling needs
-
Provide direction to field staff as needed to ensure appropriate patient coverage

Report scheduling changes and client concerns to supervisors

Verify time records against schedules

Follow up on scheduling issues reported by on-call staff

Attend required meetings and training
-
Maintain compliance with agency policies and procedures

Perform other duties as assigned
What Makes You Successful

Previous scheduling or intake experience (home health strongly preferred)

Detail oriented with excellent verbal, written, and interpersonal communication skills

Friendly, professional phone presence and comfortable handling high call volume

Strong organizational and time-management skills

Ability to prioritize and multitask in a fast-paced environment

Strong computer proficiency

Knowledge of medical terminology preferred

Ability to work independently with minimal supervision
- Willingness to work occasional weekends and holidays as required to support patient care and operational coverage
