Join Our Growing Team

Croí Health is seeking an experienced, compassionate, and detail-oriented Licensed Practical Nurse (LPN) to join our team as a Triage & Wellness Nurse.

In this vital role, you will serve as a key point of contact for patients, families, physicians, and clinical staff by managing live triage calls, coordinating communication, documenting critical information, and supporting patient wellness through outreach and follow-up calls.

Schedule: Full-Time, 40 hours per week, onsite in Norwell

Compensation: 35.00 - 42.00 per hour based on experience

How You'll Make an Impact

  • Answer and manage live triage and physician telephone lines, ensuring timely and accurate communication with clinical staff.
  • Assess incoming calls and appropriately route information from physicians, patients, caregivers, and family members to the appropriate clinical manager or team member.
  • Conduct wellness calls to patients for appointment reminders, follow-up care, patient education, and general well-being checks.
  • Coordinate, document, and communicate laboratory results to clinicians, physicians, patients, and family members as directed.
  • Serve as a communication liaison between patients, referral sources, physicians, and internal clinical teams.
  • Maintain accurate and timely documentation within agency systems.
  • Support quality assurance initiatives and maintain established service standards.
  • Assist in reviewing, analyzing, and tracking clinical referral and patient data as needed.
  • Ensure compliance with all agency policies, regulatory requirements, and quality standards.
  • Perform additional duties and special projects as assigned.

What You'll Bring to the Team

  • Current Massachusetts LPN License in good standing
  • Graduate of an accredited Licensed Practical Nursing (LPN) program
  • Minimum of five (5) years of recent nursing experience
  • Minimum of three (3) years of community health or home health experience preferred
  • Knowledge of Medicare and Managed Care regulations and processes
  • Strong organizational and time-management skills
  • Excellent verbal and written communication skills
  • Outstanding customer service and interpersonal skills
  • Ability to effectively prioritize, problem-solve, and multitask in a fast-paced environment
  • Strong analytical and troubleshooting abilities
  • Proficiency with computers, electronic medical records, and keyboarding
  • Ability to communicate professionally and compassionately with patients, families, physicians, and staff
