The primary location for this position is remote (EST or CST required). Expected schedule will be 4x10s (Monday-Thursday or Tuesday-Friday) 9:00am-7:00pm EST or CST. 1-2 Saturdays per month will also be required.


What You’ll Do

As a Resource Pediatric Registered Nurse at Imagine Pediatrics, you serve as a key clinical partner within the Patient Engagement (Outreach & Enrollment) team, supporting the re-engagement of previously enrolled patients and families. This role focuses on providing real-time clinical support to patients who are newly reconnected to services, ensuring immediate access to care, guidance, and triage at the point of engagement.

In this role, you will collaborate closely with Patient Engagement Agents, Community Health Workers, and interdisciplinary care team members, operating within a non-traditional structure aligned to patient engagement and operational priorities. You will act as a primary point of contact during re-engagement, delivering timely, family-centered clinical support through virtual touchpoints and ensuring patients are quickly connected to appropriate care pathways.

Key responsibilities include:

  • Provide real-time clinical support and triage for newly re-engaged patients, ensuring immediate response to clinical needs following outreach and enrollment interactions
  • Deliver proactive, professional, and compassionate care, serving as an accessible first point of contact for patients and caregivers
  • Apply a family-centered care approach, meeting patients where they are and supporting individualized care needs
  • Demonstrate cultural competence and sensitivity, effectively supporting diverse populations and leveraging appropriate resources as needed
  • Build trusting relationships with patients and families to support engagement, understand needs, and encourage ongoing participation in services
  • Perform a comprehensive assessment of patients’ clinical, psychosocial, discharge planning, and financial needs, establishing appropriate clinical milestones and goals
  • Collaborate with physicians, APPs, and interdisciplinary team members to develop and execute care plans aligned to patient needs and clinical goals
  • Coordinate with Resource specialists, Behavioral Health, and other interdisciplinary partners to address complex needs and ensure appropriate referrals and follow-up
  • Monitor and evaluate the effectiveness of care plans, gathering input from relevant sources to ensure care is delivered in an accurate, safe, timely, and cost-effective manner
  • Document all assessments, interventions, and patient interactions accurately and in a timely manner
  • Meet established monthly re-engagement productivity and engagement goals, including completed patient encounters, effective warm handoffs, and sustained caregiver engagement
  • Contribute to longitudinal engagement outcomes by supporting a target percentage of re-engaged patients to complete a clinical visit within three months of initial re-engagement

What You Bring & How You Qualify

First and foremost, you’re passionate and committed to creating the world our sickest children deserve. You want an active role in building a diverse and values-driven culture.   • Licensed RN in at least one state with eligibility to register for other state licensures.
  • Graduate with a bachelor’s in nursing from an accredited university preferred.
  • Minimum 1 year care coordination or case management required
  • Familiarity with Medicaid regulations and services required
  • Experience in Value Based Care highly preferred.
  • Virtual care experience a plus.
  • Bilingual Spanish is required.

The hourly rate for this position ranges from $36 - 42 per hour in addition to a quarterly performance-based bonus, competitive company benefits package, and eligibility to participate in an employee equity purchase program (as applicable).
