Veteran Pathways & Proactive Care is seeking experienced General Practitioners to deliver chronic disease management and structured multidisciplinary case conferencing for veterans and patients with complex, multi-system health needs. The role runs across two structured days each week — an in-clinic day at our Ipswich partner practice and a remote telehealth case-conferencing day — with some flexibility on the days to suit you.
This is not a standard locum. You will provide clinical leadership within integrated Chronic Care Teams, working alongside allied health professionals and specialists to translate clinical findings into coordinated, evidence-based care plans. The work is structured, purposeful and genuinely multidisciplinary — a system that values thorough, evidence-based care over high-volume throughput.
Remuneration
First 3 Months (Guaranteed) = $240.00 per hour
After 3 Months = 70% of billings
- Structured as an independent contractor arrangement.
- Bulk-billing model across Medicare and DVA billing streams.
- Sign-on bonus and anniversary incentive available (details on enquiry).
- Ipswich is a DPA-eligible location — GPs with a 19AB exemption, including international medical graduates, are welcome to apply
Location & working options
In-clinic day (Tuesday): an 8-hour clinic day at a well-established allied-health practice and a natural multidisciplinary home for chronic-care work.
Remote case-conferencing day (Thursday): an 8-hour telehealth case-conferencing session, delivered from your home office (a suitable space and reliable internet are all that’s required). There is some flexibility on which days these fall, arranged around your availability.
The Role
As a General Practitioner within our Chronic Care Team model, you will provide clinical leadership across in-clinic consultations and structured telehealth case conferencing:
- Conduct comprehensive GP consultations for acute, chronic and complex conditions within a Chronic Care Team framework (in-clinic and/or by telehealth).
- Deliver preventative health assessments — cardiovascular risk, metabolic health, mental-health screening and lifestyle risk modification.
- Undertake chronic disease management (CDM) reviews and care planning, including GP Management Plans (GPMPs) and Team Care Arrangements (TCAs).
- Lead and participate in scheduled telehealth case conferences as part of multidisciplinary Chronic Care Teams, integrating clinical, pathology, imaging and allied-health input into coordinated care plans.
- Initiate, review and manage pathology and imaging investigations; provide prescribing and medication management; and refer to allied health, specialists and community services.
- Ensure clinical safety, prioritisation and escalation pathways are clearly identified and actioned.
Working pattern: the role is built around two structured days each week — an 8-hour in-clinic day on a Tuesday at our Ipswich practice (consultations and chronic-disease reviews) and an 8-hour remote case-conferencing session on a Thursday (approximately 22 patient reviews, conducted entirely via telehealth). There is some flexibility on which days these fall, arranged around your availability.
Veteran Health & DVA Focus
Veteran Pathways & Proactive Care has a dedicated focus on veteran health and chronic disease management for both Medicare and DVA-funded patients. You will work with veterans who often present with complex, multi-system conditions including chronic pain, mental health comorbidities, musculoskeletal injuries, and the long-term effects of military service.
Experience with DVA billing, the Coordinated Veterans’ Care (CVC) Program, or veteran-specific health assessments is highly regarded but not essential — comprehensive onboarding and support will be provided.
Requirements
Essential
- Medical degree (MBBS or equivalent) and current registration with the Medical Board of Australia (AHPRA).
- Fellowship with RACGP or ACRRM (VR GP), OR eligibility for a 19AB exemption in a DPA location such as Ipswich (IMGs welcome).
- Experience in chronic disease management, preventative health and complex care coordination.
- Competence in prescribing, referrals and investigation management.
- Strong communication and collaboration skills within multidisciplinary teams.
- For the telehealth pathway: a suitable home-office environment and reliable internet connection.
Desirable
- Experience with telehealth consultations and digital-health platforms.
- Experience within team-based or multidisciplinary care models.
- Interest or experience in veteran health, DVA processes or the CVC Program.
- Experience with GP Management Plans (GPMPs), Team Care Arrangements (TCAs) and chronic disease care planning.
