Clinical Leadership & Resource Support
- Serve as the resource RN for all field clinicians, providing real‑time clinical guidance, symptom‑management recommendations, and support with complex patient situations.
- Assist staff with care‑planning decisions, medication adjustments (per protocol), and escalation pathways.
- Engage in course‑of‑action discussions with clinicians, triage nurses, and providers to ensure safe, appropriate, and timely interventions.
Intake, Admissions & Care Coordination
- Collaborate closely with the intake and admissions team to coordinate weekend admissions, evaluate appropriateness, and ensure timely response to referrals.
- Monitor and support the flow of new admissions, resumption of care visits, and urgent evaluations.
- Communicate with physicians, facilities, and interdisciplinary team members to maintain continuity of care.
Staffing, Productivity & Operational Oversight
- Monitor weekend staffing levels, productivity expectations, and visit completion to ensure adequate coverage and workload balance.
- Adjust assignments based on acuity, geography, and emergent needs.
- Provide support to field staff experiencing high caseloads or unexpected challenges.
Compliance & Quality Assurance
- Ensure compliance with agency standards, including HUV (High‑Utilization Visits) and SFV (Skilled Follow‑Up Visits)
- Review documentation for accuracy, timeliness, and adherence to hospice regulations.
- Identify potential compliance risks and communicate concerns to leadership.
Patient, Family & Facility Communication
- Serve as a point of contact for patients, families, and facility partners during weekend hours.
- Address concerns, provide education, and support emotional needs with compassion and professionalism.
- Facilitate communication between families, field clinicians, and the interdisciplinary team.
Triage & Crisis Support
- Collaborate with the triage team, especially during periods of high call volume, to ensure timely response to symptom crises, deaths, and urgent visits.
- Provide clinical backup and decision‑making support to triage nurses.
- Coordinate urgent field visits when necessary.
Leadership & Collaboration
- Maintain clear communication with weekday leadership regarding weekend events, patient changes, staffing issues, and operational needs.
- Promote a culture of teamwork, accountability, and compassionate care.
- Support onboarding or mentoring of new weekend staff as needed.
Qualifications
- Active RN license in good standing.
- Minimum 2–3 years of hospice or palliative care experience (preferred).
- Strong clinical assessment and critical‑thinking skills.
- Ability to lead, mentor, and support a multidisciplinary team.
- Excellent communication, organization, and problem‑solving abilities.
- Comfortable working independently and making high‑level clinical decisions.
- Proficient with HCHB and hospice documentation standards.
