RN Clinical Coordinator for Home Healthcare

Physicians Choice Home Health is seeking an experienced RN Clinical Coordinator to help support high-quality patient care across our home health team. In this role, you will partner closely with the Director of Patient Care Services, your RN Clinical Coordinator colleagues, and our team of field clinicians to ensure patients receive safe, coordinated, and compliant care.
This position is ideal for a nurse who enjoys supporting field clinicians, improving care quality, and helping ensure patients receive the right care at the right time.

Responsibilities:
You will support both patient care coordination and clinical team operations, including:
- Providing oversight and care coordination for a designated caseload of patients
- Supporting care coordination by working with physicians, healthcare providers, and community partners
- Responding to incoming clinical questions from field staff and patients, and escalating to the Director of Patient Care Services when needed
- Reviewing and signing clinical documentation, including orders for Starts of Care, Recertifications, Resumptions of Care, and Discharges, as well as external orders
- Helping identify and implement clinical quality and documentation improvement opportunities
- Supporting and educating new and existing staff through onboarding and training to ensure quality, compliant patient care
- Participating in after-hours/on-call support on a rotating basis
- Field visits to supervise our clinicians and/or provide patient care as needed
- Promoting a positive, supportive environment for field clinicians
- Other duties as assigned by the Director of Patient Care Services
Requirements
- Active California Registered Nurse license.
- At least 1 year of home health experience as an RN field clinician
- Ability to work in our office in Torrance, CA during regular business hours
- Current CPR certification
- Ability to travel locally to patient homes
- Strong understanding of home health documentation and clinical workflows
- Experience with Kinnser/Wellsky EMR preferred
- Clinical management experience preferred

- This role is based fully in-office, Monday-Friday, 9am-5pm
- This role also includes field work, visiting patient homes to provide clinical training, conduct supervisory visits, and provide occasional patient care
Compensation and Benefits
Salary Range
- $90,000 – $100,000 annually
