Key Responsibilities
The Hospice Post Acute Care Transition Liaison is primarily responsible for securing physician orders and coordinating services for hospital referred patients. This role serves as a liaison between the Company and external partners, including hospitals, home health and non-medical homecare providers, physicians, and other referral sources. The Post Acute Care Transition Liaison also plays a key role in promoting and educating the community and medical professionals about the Company’s full range of services. This position involves daily interaction with patients, healthcare professionals, and community organizations to ensure continuity of care, timely communication, and appropriate documentation. Working closely with internal staff, the Post Acute Care Transition Liaison ensures that patient care is managed effectively and efficiently. This is a field-based position that requires regular travel outside the office to perform essential duties.
In this role you will be responsible for:
- Identification and monitoring of primary and secondary service areas.
- Development and effectiveness of a strategic plan that addresses company home care service/product lines.
- Coordinate appropriate patient referrals by assisting physicians, inpatient facilities, and other community referral sources to better understand the quality of the company’s home care services.
- Networking with physicians and community partners to develop specialized programs and opportunities for growth in the organization.
- Develops processes to track data, analyze data and make necessary changes for growth in the organization.
- Maintains an awareness of current trends in home care by participating in educational programs and seminars.
- Participates with staff to develop and produce necessary educational materials for distribution to interested parties.
- Meets with community groups, various health care professionals and other interested parties to provide information programs on the services available to homebound patients/clients in the primary/secondary service areas.
- Partner with staff to serve as a patient advocate in response to service reports received from the community or individual patients/clients, physicians or discharge planners.
- Responds to issues involving concerns over service by meeting with management staff and assisting in problem resolution.
Required Qualifications
- High School Diploma or GED
- One to three years of sales experience.
- Proven interpersonal, coordination, and leadership skills with ability to communicate effectively.
- Practical and theoretical knowledge of hospice and palliative care.
- Demonstrates active involvement in professional organizations and community activities.
- Reliable transportation and valid automobile insurance coverage.
Desired Qualifications
- Associate's Degree preferred
- Sales experience in the medical field preferred
- Clinical licensure preferred
