About the Position:

As a Care Coordinator, you provide support and guidance to students while enrolled at Circle Tree Ranch. They facilitate access to resources, coordinate services, and ensure individualized transition planning. Responsibilities include conducting assessments, monitoring treatment plans, advocating for student needs, and maintaining accurate documentation. Care Coordinators collaborate with clinical staff, demonstrators, and external agencies to promote holistic well-being. Strong communication, crisis intervention, and problem-solving skills are essential.

### What You Will Do:
  • Coordinates and facilitates Meetings with the student’s recovery team, including health homes, probation officers, and other relevant parties to support student expression and to address any student needs
  • Develops and implements transition plans to ensure continuity of care, addressing housing, employment, education, and ongoing support needs.
  • Communicates and collaborates with the Department of Child Safety (DCS) to support students involved in the child welfare system.
  • Assists students with court-related matters, including attending hearings and providing necessary documentation.
  • Supports students in obtaining identification and other essential documents needed for employment, housing, and services.
  • ### What You Will Bring:
  • High School diploma or equivalent
  • Training or relevant work experience.
  • 3 - 5 years' experience in behavioral health case management.
  • Experience with the fundamentals of the Therapeutic Teaching Community.
  • Understand of causes, symptoms, and effects of substance abuse.
  • Valid Arizona DPS Fingerprint Clearance card.
  • Current valid CPR and First Aid certification.
  • CPI (Crisis Prevention Intervention) & ESR (Emergency Service Response) certification.
  • Proficiency with Microsoft Office Suite, Electronic Medical Record, or similar software.
  • Excellent organizational skills and attention to detail.
  • Excellent written and verbal communication skills.
  • Ability to maintain confidential information.
  • Ability to operate general office equipment.
  • Demonstrates an understanding of human emotions with a high level of empathy.
  • Ability to exercise an amount of empathy and understand the importance of role-reversal.
  • Demonstrates skill in patience, active listening, and interest in diverse life stories.
