Xtreme Care is hiring a Home Care Marketer focused on Traumatic Brain Injury (TBI) referrals across NYC and Westchester County. This is a field-based role for someone who knows how to build referral relationships, educate partners, and drive intake — not a desk job.
What You’ll Do
- Build and maintain referral relationships with:
- Hospitals & discharge planners
- Rehab facilities & neuro providers
- Case managers, social workers, and attorneys
- Present Xtreme Care’s TBI home care services
- Attend in-person meetings and community outreach events
- Track referrals and work closely with intake to convert them
What We’re Looking For
- Experience in home care or healthcare marketing
- TBI, neuro rehab, Medicaid, or waiver experience strongly preferred
- Comfortable with in-person, field-based marketing
- Strong communication and follow-up skills
- Familiarity with NYC or Westchester referral networks
