Hospice On-Call Registered Nurse- weekends

KeystoneCare is seeking compassionate and dedicated Hospice On-Call Registered Nurse to provide weekend support for our hospice patients and their families. This critical role ensures that patients receive expert care and guidance during off-hours, helping to manage symptoms, provide comfort, and support families through the challenges of end-of-life care.
As an on-call nurse, you will be a vital member of our interdisciplinary hospice team, responding to urgent patient needs, collaborating with physicians and staff, and assisting with care coordination to ensure seamless service delivery.

Primary Job Responsibilities
- Respond promptly to on-call hospice patient needs during weekends.
- Assess patient conditions, manage symptoms, and provide necessary nursing interventions.
- Provide guidance and support to patients' families and caregivers over the phone or in person as needed.
- Collaborate with the hospice interdisciplinary team to ensure continuity of care.
- Document all patient interactions, assessments, and interventions accurately and timely.
- Adhere to hospice philosophy and policies in all aspects of care delivery.
- Admit patients who are hospice appropriate and interact with families in assessing needs and treatment plan.
Requirements
- Current RN license in good standing to practice in the Commonwealth of Pennsylvania
- Experience in hospice or palliative care preferred
- Excellent clinical assessment and symptom management skills
- Strong communication and interpersonal skills
- Ability to respond promptly and independently during on-call shifts
- CPR certification
- Driver's License and reliable vehicle
