Coho Integrated Solutions is seeking a dedicated Licensed Clinical Social Worker (LCSW) – Case Manager to support Military Treatment Facilities in Okinawa and Kadena, Japan. This position provides comprehensive case management, discharge planning, patient advocacy, and psychosocial support services to ensure continuity of care and optimal patient outcomes.
The Licensed Clinical Social Worker serves as an integral member of the healthcare team, working closely with physicians, nurses, behavioral health professionals, and community resources to coordinate patient care across the continuum of services.
Responsibilities (include but are not limited to):
- Provide comprehensive case management services to patients with complex medical, behavioral health, and psychosocial needs.
- Conduct psychosocial assessments and identify barriers to treatment, recovery, and successful community reintegration.
- Develop, coordinate, and monitor individualized case management plans.
- Participate in Utilization Management and Case Management meetings to identify opportunities for improving patient outcomes and healthcare delivery.
- Collaborate with healthcare providers to facilitate continuity of care and appropriate resource utilization.
- Coordinate discharge planning activities and post-acute care services.
- Provide crisis intervention, counseling, and patient advocacy services.
- Facilitate referrals to military, community, and specialty support services.
- Work closely with RN Case Managers to develop and implement integrated Case Management Programs (CMP).
- Support development of collaborative nursing and social work strategies to improve patient care coordination.
- Deliver educational presentations and in-service training related to social work and case management topics.
- Maintain accurate documentation and case records in accordance with organizational and regulatory requirements.
- Participate in quality improvement initiatives and interdisciplinary team meetings.
Essential Job Qualifications and Requirements:
- Education and Certifications:
- Master of Social Work (MSW) degree from a program accredited by the Council on Social Work Education (CSWE)
- Required Experience:
- Certification as a Certified Advanced Social Work Case Manager (C-ASWCM) through the National Association of Social Workers (NASW), OR
- A minimum of three (3) years of experience within the preceding four (4) years as a social work case manager in a hospital-based setting performing medical crisis counseling and discharge planning
- Current, full, active, and unrestricted license as a Licensed Clinical Social Worker (LCSW) in a U.S. state, territory, commonwealth, or the District of Columbia
- Skills & Competencies
- Thorough knowledge of case management principles and healthcare delivery systems
- Strong understanding of discharge planning and care coordination processes
- Experience conducting psychosocial assessments and crisis intervention
- Knowledge of military and community support resources
- Excellent communication, counseling, and advocacy skills
- Ability to work effectively with interdisciplinary healthcare teams
- Strong organizational and documentation abilities
- Preferred Qualifications
- Experience supporting military treatment facilities or Department of Defense healthcare programs
- Certification as a Certified Advanced Social Work Case Manager (C-ASWCM)
- Experience working with complex medical and behavioral health populations
- Familiarity with military healthcare systems and beneficiary populations
- Work Environment
- Military Treatment Facility and healthcare settings supporting active-duty service members, retirees, and eligible beneficiaries
- Collaborative multidisciplinary healthcare environment
- Regular interaction with patients, families, healthcare providers, and community agencies
