Maintain meticulous patient care documentation in accordance with departmental policies and procedures.
Identify and report adverse drug reactions, interactions, and allergies, and initiate appropriate interventions as per policy. Advise physicians and nursing staff on medication availability, dosage forms, side effects, and incompatibilities. Recommend formulary alternatives to optimize patient therapy.

Qualifications:

Bachelor’s Degree in Pharmacy
Current Florida Pharmacist license
1-3 years pharmacist experience

As full time, nonexempt Staff Pharmacist with CPS Solutions, LLC at Lake Charles Memorial Hospital, you will fill orders with accuracy; check technician-prepared products before release; maintain security of pharmacy and controlled substances; enter orders and patient information with accuracy; aid in the training of new staff; update the Director about the daily activities of the pharmacy; and aid the Director in maintaining the pharmacy.

Schedule: This is a full time position working 32 to 40 hours a week. Shifts vary from 8, 10 or 12 hour day and evening shifts, rotating weekends and holidays..

About the Facility: Lake Charles Memorial Hospital is the only locally-owned, regional community healthcare system in Lake Charles. Required Qualifications:

Pharmacy degree (BS or PharmD) from an ACPE accredited school of pharmacy; PharmD preferred
Current LA Pharmacist license in good standing
Experience as a Pharmacist in a Hospital setting preferred
Ability to work independently, set priorities, make critical decisions, and respond to client concerns
Demonstrated accuracy and timeliness in a work setting
Experience using computer applications including word processing, spreadsheets, and pharmacy information systems

1. Performs assessment of patient therapy that demonstrates knowledge and consideration of age specific and cultural differences. Makes recommendations to modify therapy to meet the age specific requirements of the newborn (0 to 30 days), pediatric (1 to 12 years), adolescent (13 to 17 years), adult (18 to 70 years), and geriatric (71+ years) patients.
2. Supervises the compounding, dispensing, ordering, and proper storage of all pharmaceuticals.

• Oversees preparation of parenteral nutrition solutions, intravenous admixtures, chemotherapy, and piggyback solutions.
• Verifies unit dose carts, IV orders, piggybacks, parenteral nutrition and chemotherapy orders prepared by the pharmacy technician, intern, or another pharmacist as required.
• Verifies entry of chart orders into the computer system by the pharmacy technician or intern.
• Rectifies any missing doses or missed doses.
• Provides drug information to physicians, nurses, pharmacists, and other health professionals.
• Reviews medication profiles and monitors patient drug therapy upon entering new orders.
• Performs all other duties as needed.

Education:

Doctorate of Pharmacy or Bachelors Degree in Pharmacy with 5 yrs experience. Advanced degree (MS, MBA, MHA) and or pharmacy residency is preferred.

Experience:

Residency or 3 years experience in hospital setting.
Knowledge of regulatory and accreditation bodies, survey standards, and outcome metrics.

Must possess a valid pharmacist license as provided by the State of California and must be in good standing

Oversees the packaging labeling, and distribution of medications for all patients and employees according to established policies, procedures, and guidelines. Monitors the use of scheduled medications on the nursing units and maintains controlled inventories of medication in the dispensing areas.
Compounds and dispenses pharmaceuticals including sterile products, chemotherapy and parenteral nutrition products.
Verifies medication orders before delivery to patient care areas and ensures accuracy and completeness. Resolves discrepancies as necessary.
Reviews all laboratory data for appropriateness of medication use and recommends changes when needed. Including microbial culture and sensitivity data, toxicology, therapeutic drug monitoring, etc.
Develops and presents orientation, education and training programs for medical, nursing, hospital and pharmacy staff, as well as assisting in the educating of pharmacy students, residents, and fellows.
Educates patients and their families about specific medications.
Responds to emergency situations to assist with medication management (e.g. response to cardiac arrests)
Provides accurate and timely medication information and or teaching to healthcare professionals or patients.
Maintains the hospital formulary and acts to limit medication usage to formulary items when appropriate
Documents all clinical activities and interventions performed. Collects data, maintains logs, records and other documentation as required by the department
Communicates professional knowledge effectively in both oral and written forms to patients and other health professionals.
Participates in quality improvement and medication use review activities of the department.

We are a progressive and ever-expanding Australian-owned and operated company that provides a complete service. The Role

We're seeking a registered Pharmacist located in Toowoomba or willing to relocate to Toowoomba (relocation allowance package provided). You will be responsible for providing an outstanding patient experience while maximizing revenue goals, supporting the development of the staff, and ensuring compliance with inventory, security, and system protocols.

Previous experience with cannabis products is desirable but not needed as we will provide you with industry standard quality training and head office support.

This role is ideally Full Time, but Part Time options will be considered for the right candidate.

Key Responsibilities:

Growing and leading a team of pharmacist assistants
Build and implement systems and processes for the dispensary
Collaboration with prescribing doctors to optimize patient outcomes
Attend clinic/dispensary meetings to communicate stock availability
Educating and consulting with your patients
Dispensing medications
Ensure accurate and timely delivery of medications
Maintaining accurate customer records
Ensuring compliance with pharmacy guidelines
Stock ordering and management, ensuring balances are checked and correct
Ensuring a safe and clean working environment

Candidate requirements:

Bachelor of Pharmacy
APHRA registration to practice as a Pharmacist (with no restrictions) in Australia
Experience working as a Pharmacist and/or Pharmacy manager in a fast paced environment
Experience receiving, processing orders, dispensing S4/S8 medications
Proficiency with Z Software is viewed favourably
Ability to be trained up on new pharmacy related software to improve efficiencies
Vehicle
