About this role

As the Chicago Community Health Worker (CHW) Manager for Waymark, you will lead our community health workers in the region. Reporting to the VP of Market Operations, you will supervise and manage a team of CHWs – including helping them implement their day-to-day priorities, providing coaching and support around patient care, and managing performance to team goals. As a leader in a growing organization, you will also develop and steward community partnerships and resource information to support patient care, and impact how we design, structure, and implement the CHW experience for our employees, spanning recruitment, hiring and training, deployment, mentorship, resourcing, providing pathways to advancement, and advocating for CHWs across our organization and externally.

Responsibilities

Provide direct supervision of the day-to-day activities of CHWs in the greater Chicago area, including helping them establish and implement their schedules and priorities.
Lead weekly team huddles with a multidisciplinary team – including CHWs, licensed clinical social workers, pharmacists, and care coordinators – to coordinate patient care.

Minimum qualifications

Experience supervising teams of community health workers or related local health workforces.

The ConcertoCare Community Health Worker (CHW) serves as a key member of a multidisciplinary care team in an integrated care setting, addressing the needs of our most medically and socially complex patients. Our care teams work in concert to bring close attention to patient’s social determinants of health needs and maintain an intense focus on engaging patients and caregivers in their care. Community Health Workers employ patient activation strategies to support patient engagement, patient education and coaching, and to facilitate access to, and care navigation of, systems targeted at improving health outcomes. This position also engages patients via assisted telehealth.

The CHW role is a critical component of the care team’s work to ensure patients meet their care plan goals, and reduce unnecessary utilization of emergency departments and admission/readmission to inpatient units. The Community Health Worker demonstrates deep cultural competency, and leans into patients’ diverse beliefs, values, and social norms, and ensures care is provided in a manner that is culturally and linguistically appropriate to meet the needs of all patients served. As a key member of the market team, the Community Health Worker also assists with market activities designed to increase patient engagement and participation in ConcertoCare’s program.

Base Salary/ Wage Range $23.00 to $27.00/hour plus annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown. The Opportunity

One Medical’s High Risk team is looking for a Community Health Worker (CHW) to act as the bridge between our patients, community resources, and healthcare services in order to remove barriers and increase wellness. Reporting to the Resource Navigation Program Manager, a CHW is a patient’s advocate or liaison, accompanying patients through proactive in-person and virtual or telephone outreach to promote health literacy and increase access to resources needed to live healthier lives. As a key member of the High Risk Care team, CHWs work with patients to identify personal health goals, support care team decision making, and coordinate clinical and complementary services needed to provide high quality health care, while also improving the quality and cultural competence of service delivery. CHWs play a critical role in supporting access to community, state, and federal resources for high risk/high need patients.

The ideal candidate has high levels of flexibility, problem solving, strong communication, and an intimate knowledge of the diverse communities we serve. You are dedicated to serving the community and building meaningful relationships. You are able to manage multiple priorities while maintaining a positive attitude. You are comfortable driving throughout Denver to reach your patients in their homes and communities, spending the majority of your time in the field and the remaining in One Medical clinics.

What you’ll likely work on:

Establish and maintain strong interpersonal relationships with patients, community organizations, team members, and partners to support and coordinate services for patients
For those patients referred for CHW services, identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services
Manage resource-focused care plans as a component of a patient’s continuum of care
Form relationships with and build an inventory of local community organizations that may benefit our patients
Connect patients to state and local community resources related to housing, transportation, food, and activities of daily living among other social and physical barriers to health.
Assist patients with completion of applications for accessing eligible benefits and resources
Promote goal setting and achievement to improve patients’ quality of life and self efficacy with patients. Goal definitions are agreed upon by the care team

Peers can also be used to perform community outreach and home visits—something that may not be possible for case managers, LCCs or front desk staff due to caseloads and required office-based tasks.

Qualifications

Education and Work Experience 1. Commitment: Passion about working to improve the health and quality of life of people living with HIV/AIDS. 2. Knowledge: Must have demonstrated knowledge of HIV/AIDS and HIV medical care services in their region of NJ, along with an interest and ability to expand knowledge through training. Knowledge of regional community resources/services is important. Ability to assess clients for needs related to treatment education, risk reduction and prevention. 3. Must demonstrate experience with issues related to HIV care and treatment. Familiarity with issues that put people at risk for poorer health is also highly helpful, such as homelessness, incarceration, domestic violence, and/or substance abuse. 4. Strong communication skills are essential. Demonstrated ability to conduct sensitive, empathetic interviews and conversations that highly respect the dignity and diversity of clients. Ability to engage staff and clients in a supportive and empowering way. Ability to help manage conflict. Ability to work effectively within a team. Ability to maintain client records, extract relevant demographic and disease information and adhere to privacy and confidentiality protocol. Basic computer skills required. 5. Personal qualities including energy, intelligence, diplomacy, sound practical judgment, the ability to work independently during periods of less supervision; the ability to work effectively in a busy environment; and the interpersonal skills to work effectively in partnership with other program staff and others in the agency. Position Summary

The Guia is responsible for a panel of patients and, in collaboration with other members of a multidisciplinary primary care team, helps patients meet their preventive, chronic, and acute care needs. The Guia engages patients and encourages them to take an active role in their health by providing the tools necessary to make healthy lifestyle choices and adopt lifelong healthy behaviors. Reports on community outreach activities including problems and concerns as needed to the program Administrator.
Prepares written reports and maintains records of outreach contacts and activities.
Updates information in database from various data forms collected.
Achieves monthly, quarterly, annual enrollment goals, determined by yearly program funding.
Maintains a good working relationship with all community members to ensure that the various programs they may support are viewed in a positive manner by the community.
Continuously expands knowledge and understanding of community resources and services. Facilitates client access to community resources, including locating housing, food, clothing, transportation and providers to teach life skills, and relevant services.

What You’ll Do

The role of the Community Health Worker (CHW) is to create connections between diverse, underserved, and vulnerable populations to health and social service systems. GENERAL DESCRIPTION OF POSITION

The Community Health Worker (CHW) is committed to supporting Santa Clara Family Health Plan Community Resource Center(s) (CRC) and has a unique role in making the health of communities more equitable, especially for populations experiencing disproportionally poor health outcomes. As a trusted community member, the CHW has a deep understanding of community needs and plays an integral & multifunctional role serving as a liaison between the community and the CRC. The CHW provides outreach in community settings, screenings for Medi-Cal eligibility, and schedules one-on-one appointments for enrollment application assistance in compliance with state and federal regulatory requirements, SCFHP policies and procedures, and business requirements. The CHW supports navigation and linkages to additional resources for SCFHP health plan members and residents by referring them to the community's most appropriate programs and resources (e.g., for food, cash assistance).

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Build and maintain positive working relationships with staff, residents, community partners and governmental agencies; form and foster partnerships with safety-net and community organizations, and with government agencies.
2. Act as liaison with community residents and keep SCFHP staff informed of current community issues and interests.
3. Provide assistance for residents and families in completing new/renewal applications for Medi-Cal and for Covered California, by phone or in-person; educate residents on eligibility criteria, obtain personal information and correctly complete forms and submit all required supporting materials.
4. Represent SCFHP and the CRC at outreach events, including health fairs, enrollment events, school presentations, and community-based organization functions.
5. Contribute toward developing a community outreach planning effort to reach residents where they live, work, pray and play.
6. Assist residents and families to feel comfortable engaging with health and safety-net services while addressing their fears from personal, community, and system barriers.
7. Through outreach activities, engage designated populations about the CRC and related services, using best practices for engaging marginalized and high-risk individuals and communities.
8. Continuously expand knowledge and understanding of community resources, services, and programs to identify service gaps; work to reduce cultural and socioeconomic barriers between residents and institutions; make recommendations for programs and initiatives to address identified issues and gaps.
9. Support residents’ and families’ navigation and linkage to resources; conduct intake interviews to identify barriers to health and health equity; solicit residents’/families’ suggestions for improving their health; using “warm hand-off,” link them to appropriate resources (internal, external partners); follow up on referrals.
10. Hold workshops and gatherings with health plan members and residents to improve health awareness and healthcare access.
11. Contribute to the development and maintenance of resource information and materials.
12. Act as interpreter when supporting non-English speaking residents.
13. Organize, coordinate, and maintain records of all activities, including completing all supporting data entry.
14. Prepare daily, weekly, and monthly reports on the progress and status of service delivery.
15. Work collaboratively and effectively within a team.
16. Identify issues, trends, and opportunities to improve efficiency and/or quality, or to better assist CRC patrons, report relevant information to management.
17. Attend appropriate staff and in-service meetings.

Required skills and experience:

Summary:

The Community Health Worker I (CHW) is developing skills to help patients, and their families navigate and access health and community services while adopting healthy behaviors. The CHW I work as a care team member through an integrated approach to care management and community outreach. The CHW I is developing skills to effectively promote, maintain, and improve the health of patients and their families by providing social support and informal counseling. 4) Assist with medication adherence, reminding patients about upcoming appointments, arranging transportation or other community services patients may need

6) Assist in identifying and utilizing community resources using an approved community resource database and demonstrate the use of applicable EMR systems. Assist in scheduling and accompanying patients to appointments as needed and assisting with the completion of applications for programs for which they may be eligible.

7) Communicates between providers and clinical team; work collaboratively and effectively within a multidisciplinary team

8) Build and maintain positive working relationships with the clients, providers, supervisors, and all staff

9) Continuously expand knowledge and understanding of community resources and services, public health prevention, and evidence-based intervention programs provided

10) Effectively utilizes and documents all activities within the EMR and community service plans, resulting in appropriate data collection tools in a clear and concise manner

11) Complete Care Needs Screenings and other applicable screenings to assess the patient's needs and provide appropriate referrals. Coach patients in the effective management of their chronic conditions and self-care

12) Implement culturally and linguistically tailored educational and support activities

Community Health Worker (CHW) will with specific target populations on identifying and address Social Determinants of Health (SDoH). CHWs will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes.

Under the direction of the manager of Behavioral Health Integrated Services, the CHW will provide patient support and provide specialized and focused assistance and other duties as assigned at all Fenway Health clinical locations.

Principal Duties and Responsibilities:

Care Coordination

Provides coverage of care coordination duties as needed
Assists patients with completion of paperwork
Provide patients with updated and accurate information regarding care coordination services
Actively build good working relationships with clients and colleagues
Assists patients with applications for enrollment in programs as needed
Document in EHR for each patient interaction
Assists department with care coordination administrative tasks such as completing and faxing client paperwork
Assist with outgoing phone calls to community resources as needed
Assists with distributing program materials to clients via mail as needed
Convey the purposes and services of a program to the patient population and the impact that program or service would have
Help patients develop health management plans and goals
Follow-up with health management/care plans with both patients and providers
Coach patients in effective management of their chronic health conditions and self-care
Assist patient in understanding care plans and instructions
Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place
Establish positive, supportive relationships with participants and provide feedback
Assist patients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services
Facilitate communication and coordinate services between providers
Motivate patients to be active, engaged participants in their health
Build and maintain positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff.
Continuously expand knowledge and understanding of community resources, services and programs provided; human relations and the procedures used in dealing with the public as part of a service or program; volunteer resources and the practices associated with using volunteers, operations, functions, policies and procedures associated with the department or program area, procedures and resources available to handle new, unusual or different situations.

The Opportunity

CHWs are expected to work within their scope of practice. There is no expected clinical license for this position.

What you’ll likely work on:

Establish and maintain strong interpersonal relationships with patients, community organizations, team members, and partners to support and coordinate services for patients
For those patients referred for CHW services, identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services
Manage resource-focused care plans as a component of a patient’s continuum of care
Form relationships with and build an inventory of local community organizations that may benefit our patients
Connect patients to state and local community resources related to housing, transportation, food, and activities of daily living among other social and physical barriers to health.
Assist patients with completion of applications for accessing eligible benefits and resources
Promote goal setting and achievement to improve patients’ quality of life and self efficacy with patients. Goal definitions are agreed upon by the care team
Meet with patients in patient-centered and patient-preferred locations (One Medical offices, patient’s home, external medical provider facility, community setting)
Support care team decision making through participation in interdisciplinary team meetings
Document interactions with patients in electronic medical record in a timely manner while maintaining HIPAA standards and confidentiality of protected health information
Manage time, set priorities, work independently, and collaborate effectively with an interdisciplinary medical team

What you’ll need:

At least 2 years of patient facing work experience in social services, community-based case management or healthcare outreach program
Strong verbal and written communication skills; Spanish Speaking preferred
Proficient computer skills (Google Workspace - Docs, Sheets, Slides) as well as experience utilizing electronic medical record systems
Must have access to an independent means of safe and reliable transportation as well as the ability to travel to various locations in the community throughout the day; mileage will be reimbursed at the IRS/Federally established rate
Community Health Worker certification or advanced education in social services is a plus

This is a full-time, hourly position based in Phoenix, Arizona that will spend a balanced time in the field and in One Medical clinics.

Job Description

General Summary

Principal Responsibilities and Tasks

Recruits a client base within the community by identifying, locating, interviewing and screening individuals who may be appropriate for the Baltimore City Cancer Program (BCCP).
Provides general information to individuals and families on program objectives and services, eligibility requirements and benefits, confidentiality of information, etc. Distributes informational materials and literature.
Schedules clients for appointments with health care providers. Reminds them of pending appointments and contacts them to inquire into reasons for missed appointments. Escorts clients to various appointments to ensure compliance and provide support.
Assists clients in obtaining necessary transportation and/or childcare when treatment is needed.
Assists with client retention by following up on all contacts by telephone or by a home visit follow-up. Locates clients who have moved or lost contact with the program.
Serves as a liaison between the client and community resources including department staff, City, State and Federal social services agencies.
Conducts visual inspection of the physical condition of the client’s house to identify factors that may be detrimental to maintaining a safe, healthy and comfortable living environment.
Assists with the facilitation of the family health center process by greeting clients and making sure that they are registered to be seen.
Assists with planning, organizing and implementing community special events such as health fairs, workshops, etc.
Reports on community outreach activities including problems and concerns as needed to the program Administrator.
Prepares written reports and maintains records of outreach contacts and activities.
Updates information in database from various data forms collected.
Achieves monthly, quarterly, annual enrollment goals, determined by yearly program funding.
Maintains a good working relationship with all community members to ensure that the BCCP is viewed in a positive manner by the community.

Qualifications

Education and Experience

High School Diploma or equivalent (GED) is required.
Working experience providing outreach services. Experience should include writing reports and maintaining records.

Knowledge, Skills and Abilities

Effective oral and written communications skills.
Ability to establish and maintain effective working relationships.
Ability to handle sensitive and confidential matters with discretion and tact.
May be required to use personal automobile while conducting official business. Must possess a valid Maryland Non-Commercial Class C or Commercial Class B Driver’s license (CDL) if required to drive.
Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient population(s) served. Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient’s requirements relative to his or her population-specific and age specific needs.

Job Summary

The Community Health Worker (CHW) works in a population-based practice setting in Clark County. The Community Health Worker, a paraprofessional classification in the Infectious Disease Program, provides culturally competent community/public health services. The majority of health and outreach services are provided off-site in client homes, at worksites, and at other community locations according to protocols and under the oversight of a Public Health Nurse (PHN), Program Manager, and the Health Officer.

Organizational responsibilities include understanding and promoting the public health mission of the Department; providing courteous, respectful, efficient customer service to all Health Department clients; honoring diversity of all department employees and constituents; participating in Health Department training; and striving for personal excellence in public health work. This position reports directly to the Program Manager of the Infectious Disease Program. CHWs working in the Infectious Disease program are to be knowledgeable about the Council on Linkages Public Health Competencies and will incorporate competencies into their public health work.

This is a full-time position, represented by Local 335 – LIUNA (Health Care Division, Laborers International Union of North America).

This position will work an 8-hour schedule Monday through Friday.

This position has the option of a remote/hybrid work schedule possibility. However, the candidate selected must reside in OR or WA. No Exceptions.

Qualifications

KEY OR TYPICAL TASKS AND RESPONSIBILITIES

Duties may include but are not limited to the following:

Demonstrates proficiency in using various computer applications (e.g. MS Word, Excel and PowerPoint) as applicable to design or complete forms, correspondence, spreadsheets, data entry, etc.
Effectively operates complex equipment and office machinery, which includes computer, printers, scanners, and facsimile machines.
Provides courteous customer service to providers, clients, and the public. Responds to routine inquiries by phone or face to face; listens to questions and explains procedures, rules or functions according to relevant program guidelines; fulfills requests for information or directs clients to appropriate person or section.
Participate with PHNs in the development of tuberculosis (TB) case management plans and actively participate in weekly review of current TB caseload.
Assist with field work related to TB case management services (i.e. specimen kit drop-off and pick-up; conducting directly observed therapy, scheduling client visits).
Communicate to PHN any observations or concerns from the client related to adverse symptoms of medication, non-compliance to TB treatment or isolation.
Assist with TB contact investigations.
Support case management strategies that assist clients to adhere to their TB treatment.
Package and ship specimens in accordance with requirements for shipping biological substances.
Maintain specimen collection, shipping and personal protective equipment (PPE) supplies and inventory.
Assist in the preparation of necessary reports, records and other documentation.
Participate in program specific or department program evaluation activities.
Perform other related duties as requested by the PHN or manager.

Education and Experience:

High school diploma or GED

2 years of experience or training in a health-related setting is desirable. Some positions may require oral and written fluency in both English and the language of the community being served. Bilingual premium pay may be available for the employee meeting bilingual criteria per County policy.

Any satisfactory combination of education, experience and training which ensures the ability to perform the work will be considered.

Knowledge of: Principles, policies, procedures, methods, and techniques specific to general health, infectious disease control, prevention, and public health; communicable disease etiology, manifestations, progression, diagnosis, and treatment; medical and drug terminology; laboratory testing procedures; community resources; confidentiality guidelines; traditions, customs and health beliefs of the community being served.

Ability to: Operate equipment for measurements of health indicators as required by program; learn and effectively communicate orally and in writing basic public health information to diverse populations; oral and written fluency in a non-English language may be required for some positions; establish and maintain effective working relationships with clients, staff, colleagues, key stakeholders, community groups, other professionals, and the general public; provide culturally and linguistically appropriate services to persons of various cultures and backgrounds; maintain records; operate computer software and other office equipment; exercise initiative, tact, discretion, and judgment in carrying out position responsibilities; manage work time appropriately and work in unstructured and unconventional work settings; prioritize tasks, interpret and apply rules and instructions.

Other Requirements:

Must have or be able to obtain Washington State Medical Assistant - Phlebotomist certification within first year of hire.
Pass respiratory mask fit-testing and qualify for the use of standard Department respiratory masks within 2 months.
Must adhere to OSHA/WISHA guidelines, including but not limited to timely completion of mandatory trainings.
Must adhere to the Department employee immunity policy and provide documents as requested.
Some positions may require verbal and/or written proficiency in a second language.
Must successfully pass a criminal history background check which may include state or national fingerprint records check, pursuant to RCW 43.43.830.
Driving is required for County business on a regular basis or to accomplish work. Incumbents must possess a valid driver's license and possess and maintain current auto insurance and an acceptable driving record throughout the course of employment.
Must be able to respond to public health emergencies or exercises at any time, except while on scheduled vacation or other leave. In addition, leaves may be cancelled under public health emergencies.
Must protect the privacy and security of protected health information as defined in State and Federal law.
