Position Summary

About This Role

As a Community Health Worker, you will be Waymark’s frontline presence in the community, connecting with patients to provide social support, advocacy and navigation. You will work to improve the health of patients from low- income communities, by connecting them with various providers and resources. You will be part of a care team that is multidisciplinary and includes licensed clinical social workers, pharmacists, and care coordinators.

Key Responsibilities

Attend a 2-3-week long paid training program.
Meet patients in the community, in the home, and in healthcare facilities and conduct a needs assessment, including motivating patients to set and achieve health goals.
Help patients with health-related social issues like homelessness, substance use and hunger.
Work with other CHWs and staff to create a directory of community resources (e.g., food banks, housing assistance programs, childcare resources, etc.).
Manage relationships with the healthcare facility providers and partner with care delivery team (Pharmacists, Social workers, and Care Coordinators).
Continuously expand knowledge of community resources, services, and programs available to members and build ongoing relationships with these organizations to advocate for members.
Accompany members to medical appointments as appropriate.
Navigate technology systems to document each patient encounter in detail and accurately.
Meet patients virtually, by phone or video visit, for conversations as appropriate.
Support outreach to individuals eligible for Waymark support both in-person and by phone to establish a relationship and let them know about Waymark’s services.
Participate in weekly care team huddles.

Study blueprints, sketches, models, or specifications to plan sequences of operations for fabricating tools, dies, or assemblies.
Verify dimensions, alignments, and clearances of finished parts for conformance to specifications, using measuring instruments such as calipers, gauge blocks, micrometers, and dial indicators.
Visualize and compute dimensions, sizes, shapes, and tolerances of assemblies, based on specifications.
Set up and operate conventional or computer numerically controlled machine tools such as lathes, milling machines, and grinders to cut, bore, grind, or otherwise shape parts to prescribed dimensions and finishes.
File, grind, shim, and adjust different parts to properly fit them together.
Fit and assemble parts to make, repair, or modify dies, jigs, gauges, and tools, using machine tools and hand tools.
Conduct test runs with completed tools or dies to ensure that parts meet specifications, making adjustments as necessary.
Inspect finished dies for smoothness, contour conformity, and defects.

Minimum Requirements

High School diploma or equivalent though graduation from a technical school is preferred.
Complete a certified Apprenticeship program whereby the employee can demonstrate experience using various tools, including but not limited to knowledge of machines and tools, including their designs, uses, repair, and maintenance.
Must be able to read and interpret drawings, follow routers, use simple math to perform basic calculations, and use basic handheld measuring tools.

Required Qualifications

• At least 1 year of health care experience, preferably working with underserved or special needs populations with varied health, economic and educational circumstances, or equivalent combination of relevant education and experience.
• Community Health Worker (CHW) certification may be required for certain states (dependent upon contractual requirements).
• Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job related travel requirements, unless otherwise required by law.

Position Summary

Our customers rely on us to help solve their challenging automation problems, including reducing costs, increasing efficiency, ensuring quality, monitoring and controlling processes, and safeguarding employees. The Area Sales Manager acts as the factory representative who facilitates the sales, marketing and applications of Banner products within an assigned regional territory.

This position reports to the Regional Sales Director.

Job Description

SUMMARY: To travel to offsite locations throughout the coverage area to perform visual inspection and analysis of grain to determine a grade in accordance with Eurofins Grain Inspection SOPs and USDA grain standards on multiple classes of grain or commodities; to perform mycotoxin and other testing procedures on a variety of commodities.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Ability to work varying schedules; notifications can be on short notice; workday can vary in the number of hours required to complete an assignment. Overnight stays at the company’s expense may be necessary on occasion.
Inspect and grade grain samples accurately and in a timely manner.
Perform tests in accordance with accepted industry practice. Test samples of grain using visual procedures and computerized equipment; test sample for weight, moisture, foreign material, and damage; including mycotoxin and falling number.
Process necessary paperwork required by the Client Company and Eurofins Grain Inspection Manager. Complete appropriate record keeping accurately and legibly.
Responsible for accuracy in reporting results; record test results and provide information to client in the form of a Grain Inspection Certificate. Responsible for timely reporting of billing information to the Rail Inspection Manager in the form of a Rail Report.
Demonstrate good verbal and written communication skills. Maintain regular communication with administrative staff regarding job status, progress, problems, etc. Relay information to the Rail Inspection Manager for proper action.
Maintain grain sampling equipment properly and ensures it is in good working order.
Represent Eurofins Grain Inspection to Client Company by acting as a liaison between facility and Eurofins Grain Inspection. Exhibit excellent interpersonal and communication skills to establish and maintain effective working relationships with subordinates, peers, management, and customers.

At OhioHealth at Home Together with Compassus, the Home Health Licensed Practical/Vocational Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Home Health Registered Nurse Case Manager. S/he services are furnished in accordance with agency policies.

Position Specific Responsibilities

Implements/follows the nursing care plan for each patient.
Provides nursing services, treatments, and diagnostic and preventive procedures as assigned.
Initiates preventive and rehabilitative nursing procedures as appropriate.
Observes signs and symptoms and reports to the physician and Registered Nurse Case Manager (RNCM) reactions to treatments, including drugs and changes in the patient’s physical or emotional condition.
Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
Evaluates with the Home Health RNCM the effectiveness of the LP/VN’s nursing service to the patient and family under the guidance of the RNCM.
Maintains accurate and complete records of observation, treatments, and care of patients.
Participates in medical record audits as assigned.
Attends staff meetings, patient care conferences, and in-services as scheduled.
Is responsible for submitting any changes in schedule to the Director of Clinical Services/Clinical Supervisor on a daily basis; participating in patient care conferences to discuss the need for involvement of other members of the health team, such as physical and speech therapists.
Prepares clinical and progress notes and submits them within 24 hours of visit.
Assists the physician and Home Health RNCM in performing specialized procedures.
Prepares equipment and materials for treatments.
Observes aseptic techniques as required.
Assists in patient learning and appropriate self-care techniques.
Ensures that all care is provided with respect for patient rights.
Reports all grievances and complaints made by patients or families to the appropriate persons.
Reports all allegations of patient abuse and/or misappropriation of patient's property as appropriate.
Follows standard precautions and infection control procedures.
Reports all accidents and incidents observed.
Identifies and responds appropriately to emergency situations.
Observes the safety needs of the patients.
Provides documentation of patient's visit on the clinical record accurately, timely, and appropriately per agency policies.
Develops treatment appropriate to patients’ disease progression in conjunction with the Home Health RNCM.
Addresses patients’ care needs with families and significant others as appropriate.
Maintains confidentiality of necessary information.
Maintains proper documentation for billing per policy.
Interfaces with patients, families, and staff to ensure customer satisfaction.
Communicates effectively with patients, families, and other healthcare providers.
Regular attendance is an essential function.
Performs other duties as assigned.

Education And/Or Experience

Certificate of an accredited program of Practical or Vocational Nursing as approved by the state required.
Two (2) years of experience as an LPN in a clinical care setting preferred.
Experience in skilled nursing, preferably in hospice, home health care, oncology or emergency care preferred.

Responsibilities

Facilitates and manages patient referrals to therapy and psychiatry. Contacts, screens, triages, and connects patients to both internal and external behavioral health resources. Maintains in-house waitlist if appropriate.
Maintains mental health provider schedules with a focus on optimizing productivity. Fills provider schedules and manages templates.
Conducts warm hand offs of patients from medical providers to facilitate integration of care, to connect high risk/high priority patients to behavioral health services, and/or for patients who have a positive screening for unhealthy alcohol use.
Assists in establishing and maintaining smooth functioning of the site’s Behavioral Health Team. Actively participates in behavioral health meetings.
Supports behavioral health patient panels by making reminder calls, tracking patient attendance, and following up on no-shows to support and improve continuity of treatment.
Supports behavioral health providers in having patients complete screening questionnaires and other paperwork.
Tracks patients’ progress and alerts providers to need for outreach and for enhanced services.
Assists in coordinating and support behavioral health patient groups.
Ensures appropriate consent and parental involvement for pediatric patients in behavioral health (at sites serving pediatric patients).
Provides basic support to patients in connection to patient portal and video telehealth visits.
Contacts community agencies and maintains updated resource lists of BH resources in the community.
Responsible for data collection, entry and generation of reports.
Participates in special projects under direction of supervisor.

Job Requirements:

Bachelor’s Degree in Social Work, Health or Human Services field or equivalent combination of education and/or experience.

Previous work providing services to persons with mental health disorders, substance use disorders, physical health conditions and/or disabilities, and/or who are experiencing homelessness.
Proficient in Microsoft Office with ability to manage databases.

Summary:

The TCO Logistics Automation Specialist is responsible for translating business requirements into new features in the Maersk Warehouse and Distribution automated systems. A Brief Overview
The Community Health Worker serves as a bridge between the community, health care and the social service systems. The Community Health Worker acts as a liaison by helping individuals, families, groups, and the communities develop their capacity and access to resources. Summary

Under the supervision of the Director of Community Resource Center the Community Health Worker – Enhanced is responsible for providing intensive case management and coordinating services to address members’ complex health-related social needs (HRSN). Community Health Worker – Enhanced serves as the primary point of contact for referred members, develops Social Care Plans, and ensures the coordination and sustainability of services within the Social Care Network (SCN).

Receive and review referrals in Findhelp from Navigators or other SCN entities.
Determine agency capacity to accept referrals and respond accordingly.
Document acceptance, rejections, and referrals to other agencies in Findhelp.
Conduct a minimum of three outreach attempts within five (5) business days.
Document all outreach efforts in the Outreach Tracker in Findhelp.
Serve as the primary point of contact for members upon successful outreach.
Conduct a comprehensive assessment of member needs and priorities.
Develop and maintain individualized Social Care Plans
Collaborate with internal and external service providers to coordinate care.
Track care management units of service and reassess eligibility for enhanced services as required.
Maintain regular contact with members to monitor progress toward goals.
Provide advocacy, support, and coaching to promote self-sufficiency.
Support members in sustaining services and natural supports post-engagement.
Attend staff meetings and formal/informal training sessions inside and outside the agency.
Participates in community activities as representative of the program when assigned.
Is available to work outside regularly scheduled work hours as needed.
Any other duties may be assigned by the immediate supervisor.

Requirements

Authority:

The employee exercises initiative and some independence in establishing his/her schedule for meetings with participants, other caseworkers and community agencies. Follows pre-established procedures for program implementation and daily operations. The individuals must be resourceful and able to assess situations to determine if a crisis is involved and how to best proceed within the framework of the agency’s as well as the program’s policies and procedures. The employee will consult with supervisor if in doubt prior to proceeding.

Qualifications

High School Diploma or equivalent required or associate’s degree in human services, preferred.
One (2) year of experience in case management, health care coordination, or community health preferred.
Bilingual (English/Spanish) strongly preferred.
Strong organizational, documentation, and data-entry skills with attention to detail.
Ability to engage with diverse populations professionally and empathetically.
Strong communication, organizational, and problem-solving skills.

? Able to provide own transportation and have a valid good standing NYS driver’s license
