Summary:
The Case Manager is responsible for providing comprehensive supportive services to HIV-positive adults utilizing a non-judgmental, low-threshold, motivational interviewing interventions in order to support client in obtaining or maintaining treatment adherence and viral suppression.
Responsibilities:
- Individual counseling regarding viral suppression and correlation of medication adherence; and explore obstacles to adherence. Reviewing and educating clients about understanding their labs and medication to achieve and maintain viral load suppression.
- Develop individualized client service plans; conduct ongoing service needs assessments and reassessments as required by contract.
- Refer clients to other support or medical providers offering a level of care appropriate to individual need.
- Connect clients to medical care, promote treatment adherence, and collaborate with providers to ensure a continuity of care.
- Facilitate case conferencing with providers to promote treatment adherence.
- Act as liaison between program and medical providers on behalf of client (with consent) in order to obtain medical labs and plan any interventions needed to support clients adherence to treatment and engagement with medical provider.
- Provide mDOT services to clients that require the support with medication adherence.
- Document service provisions according to agency standards using electronic client information system.
- Performs other department or agency related duties as directed by supervisor.

Qualifications
Knowledge:
This position requires a BA with 2-4 years direct client service experience. Bilingual Spanish/English preferred. A demonstrated knowledge or HIV/AIDS, Hepatitis C, and application of harm reduction theory and practice (preferred). Training in Motivational Interviewing preferred. Must have exceptional communication skills, both orally and in writing, with individuals and groups regarding complex and sensitive issues and regulations. The physical ability to perform the essential job functions is needed. In addition, the Case Manager shall perform any duties as required to meet the needs of the program and the clients we serve.
