Position Summary:
Reporting to the Clinical Manager of Pharmacy Services you will be an integral member of the Pharmacy team and be accountable for the development of pharmacists on your team as well as clinical patient care and distribution activities. You will collaborate with the pharmacy leadership team and work with staff to determine their clinical, technical, teaching and research related needs. With the pharmacy team, you will facilitate and implement processes to ensure these needs are met. You will also contribute to peer, pharmacy undergraduate, interdisciplinary and pharmacy residency teaching/training and be an active participant in relevant inter-professional hospital committees. you will be required to travel between the three hospital sites.
Key Qualifications:
- Baccalaureate Degree in Pharmacy or Pharm D.
- Licensed in Ontario to practice pharmacy (Part A of Register)
- Completion of hospital pharmacy residency or equivalent experience is preferred; equivalent experience is interpreted as three years of experience in a broad range of pharmacy operational, clinical and/or administrative pharmacy practice
- Minimum 3 years of recent experience in hospital pharmacy practice required
- Experience being a preceptor for students or equivalent training and experience
- Completion of leadership training is an asset
- Active membership in the Canadian Society of Hospital Pharmacists (CSHP) is an asset
- Strong leadership, interpersonal, problem solving, organizational and communication skills
- Experience in providing exceptional patient centred care
- Experience in hospital pharmacy systems and distribution systems are an asset.
- Ability to work in a fast paced environment both independently and cooperatively with the team

Responsibilities include but are not limited to:
Professional Development and Teaching
- Develop, implement, assist and evaluate training processes, continuous education and knowledge translation for pharmacy staff
- Act as a mentor and resource for staff
- Active involvement in teaching pharmacy, medical and nursing staff and/or other health care professionals and students
Clinical and Distributive Services
- Review and participate in enhancing comprehensive pharmacy clinical programs
- Collaborate with Medication Management Pharmacist to review distribution processes training and development
- Provision of comprehensive pharmaceutical care to patients
- Clinical care of patients in comprehensive clinical programs including warfarin dosing, IV to PO, aminoglycoside and vancomycin dosing.
- Use of Pharmacists’ clinical scope of practice as per THP’s Policy and Procedure to optimize patient care
- In collaboration with other clinical pharmacists, deliver appropriate teaching to Pharmacy residents and Pharmacy students
- Participate in pharmacy case presentations, Pharmacy Education Rounds, journal clubs, case discussions and research projects
- Drug distribution responsibilities
- Collaborate with physicians, nurses and allied health team members to provide optimized patient care
- Other duties as assigned including the ability to potentially work at all Trillium Health Partners sites
Administrative and Human Resource Management
- Assist in the scheduling of staff to ensure coverage of clinical patient care activities and drug distribution activities at the respective site
- Assist the Clinical Manager in the process of performance appraisals for all pharmacists
- Conduct regular pharmacist team meetings
- Assist Clinical Manager of Pharmacy in the planning, organizing and directing of pharmacy policies and procedures in accordance with hospital initiatives, professional practice standards and provincial and federal laws
- Works in conjunction with other Resource Pharmacists and the Clinical Manager cooperatively to prioritize, coordinate and integrate workloads
Professional Activities and Research
- Promote Pharmacy Services by participating on relevant hospital committees and professional committees
