Clinical Pharmacist,  Family Health Center

Valley Health Partners, Allentown, PA

Provides support to VHP physicians and advance practice clinicians in areas related to medication use and quality initiatives. Provides medication and disease state management through Collaborative Practice Agreements for chronic disease states. Completes drug information requests and participates in educational efforts related to medication use. Identifies, organizes, and completes performance improvement projects related to medication use.

Qualifications:

  • Education - Required
  • Bachelor's Degree Pharmacy or
  • Doctorate Pharmacy 
  • Experience - Required
  • 3 years relevant pharmacist experience or Completion of PGY-1 residency
  • Preferred- 5 years relevant pharmacist experience; Completion of PG-1 and PGy-2 residency
  • Licenses - Required
  • Registered Pharmacist
  • Certifications
  • BCPS - Board Certified Pharmacotherapy Specialist or other relevant BPS certification completion within 2 years of hire. Preferred upon hire.
  • Authorization to Administer Injectables – Pennsylvania required
  • Basic Life Support (BLS) required
  • Knowledge, Skills, and Abilities - Required
  • Ability to accurately monitor medications.
  • Ability to work effectively in a team environment.
  • Ability to make independent judgments under stressful situations at times.
  • Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.

Additional Responsibilities:

  • May conduct in person or virtual patient encounters.
  • May evaluate for and refer to Enabling Services resources within the health center.
  • May represent pharmacy on multidisciplinary health center committees and councils.

Essential Functions:

  • Evaluates patient medical profiles for appropriateness of medication therapy based upon patient specific medical history, age, and physical status, and makes/documents drug therapy recommendations.
  • Supports patient care functions of the health center, as part of a collaborative multi-disciplinary team.
  • Ensures quality of care through formulary and/or medication management and quality improvement projects.
  • Demonstrates a continual enhancement of clinical knowledge, professional development, and promotion of the pharmacy profession through training staff and/or students and residents.
