This position is reliant upon funding and may be subject to modification or termination based on resource availability.

POSITION SUMMARY:The Case Manager provides comprehensive support to refugees and other vulnerable populations from arrival through their first five years in the U.S., helping them access resources and achieve self-sufficiency. This role requires compassion, organization, and cultural competence to navigate complex systems and advocate for clients’ needs.

Role & Responsibilities:

  • Implement program guidelines to ensure comprehensive, client-centered case management, including public benefits applications like SNAP, TANF, and Medicaid, navigating healthcare systems, and coordinating referrals to community services.   
  • Provide ongoing case management support to clients, including regular follow-ups to monitor progress and adjust service plans as needed.     
  • Act as a self-sufficiency coach to guide clients on their journey, teaching them how to find resources independently, educating them on their rights and responsibilities in the U.S., and empowering them to make informed decisions.     
  • Work closely with internal programs like employment and health and wellness programs, as well as with case managers, program specialists, and volunteers, to deliver holistic support to clients.     
  • Advocate on behalf of clients with service providers, landlords, and other community resources.     
  • Identify and establish partnerships with local community organizations and service providers to expand available resources for clients.     
  • Coordinate external relationships and referral systems to support client needs and integrate services across programs like employment, career advancement, immigrant connections, and education or ESL programs.     
  • Maintain accurate and up-to-date case files, including detailed case notes on client interactions, service plans, and outcomes. 
  • Assist clients in navigating legal and administrative systems, including applying for benefits, paying off travel loans, making immigration service referrals, and updating addresses according to U.S. Citizenship and Immigration Services policies.     
  • If necessary, help coordinate or transport clients to mainly urgent medical appointments they may not be able to get to on their own and offer translation services if applicable.     
  • Attend all staff meetings as required.    

Job Requirements:

  • Able to affirm and/or acknowledge World Reliefs Core Beliefs, Statement of Faith, Christian Identity and National Association of Evangelicals' For the Health of The Nation document
  • Bachelor’s degree in social work, human services, or a related field preferred.     
  •  Experience in case management, health education, or related fields required; experience with refugee or vulnerable populations preferred.     
  •  Highly organized, with the ability to manage multiple cases simultaneously and prioritize tasks effectively.     
  • Strong understanding of the challenges faced by refugees and immigrants, including trauma-informed care and cultural sensitivity.     
  • Excellent verbal and written communication skills, with the ability to clearly convey information to clients, colleagues, and external partners.     
  • Strong interpersonal skills, including empathy, active listening, and conflict resolution.     
  • Fluency in English required; proficiency in a second language like Spanish, Arabic, Swahili, Spanish or others highly desirable. 
  •   A valid US driver’s license and clean driving record are required.
