Based at our Community Hub, this role will be a visible and trusted first point of contact for residents, providing triage, assessment, short-term intervention and referral. You’ll work flexibly across programs, including the Community Health Seniors Frailty Program and other emerging initiatives, helping people stay well and avoid unnecessary hospital visits.

• Build and maintain trust with local residents, acting as an approachable presence in the Community Hub
• Collaborate with allied health, nursing, and community engagement teams to coordinate care
• Contribute to evaluation and continuous improvement of the Community Paramedicine model
• Provide operational and clinical leadership, mentoring early-career staff and students
About You

- A deep understanding of the social determinants of health
- Strong communication and engagement skills, particularly in diverse communities
Experience in community health or primary care will be highly regarded.
