Desert Regional Medical Center is a 385-bed acute-care hospital classified as a stroke receiving center and Level I Trauma Center with an innovative, patient centered and evidence-based Rehabilitation Services Department. Our compassionate team provides a wide range of inpatient and outpatient services, including acute care rehabilitation, joint replacement & spinal surgery, neurosurgery, ICU, Telemetry, step-down care, skilled nursing, as well as outpatient therapy, hand and lymphedema clinics.
GENERAL DUTIES:
The Patient Safety Officer(s) will have primary oversight of the facility-wide patient safety program. This leadership role will direct others within the facility towards process improvements that will support the reduction of medical/health care errors and other factors that contribute to unintended adverse patient outcomes. This practitioner provides leadership for safety assessments, coordinates the activities of the patient safety function or committee, educates other practitioners on the system-based cause for medical errors, consults with management and staff, and communicates literature-based ideas regarding effective patient safety strategies to others within the organization. Key responsibilities of this position also include working with all services to hardwire reliable delivery of healthcare systems that incorporate best practice patient safety methodologies.

DEPARTMENT SPECIFIC DUTIES:
1. Oversees the Patient Safety Program and the Patient Safety Committee, including the grievance process.
2. Investigates patient safety issues within the organization.
3. Leads Root Cause Analysis, Focused Reviews, and Intense Analysis.
4. Supports and encourages error reporting, including near misses, throughout the organization through a nonpunitive reporting system.
5. Ensures compliance with audits and monitoring defined in Action Plans.
6. Identifies reportable events and collaborates with the DCQI in order to comply with five-day reporting requirements for California Department of Public Health (CDPH) and all other regulatory requirements.
7. Notifies the Chief Medical Officer and Corporate leadership of potential risk issues.
8. Works collaboratively with hospital leadership to promote best practices for patient safety and ensure compliance with patient safety initiatives.
9. Revises and/or develops policy and procedures involving patient safety.
10. Incorporates a mechanism for internal communication of patient safety related information.
11. Designs and implements educational presentations that facilitate the understanding and implementation of patient safety standards within the organization.
12. Serves as a resource for employees and medical staff on issues of patient safety.
13. Works collaboratively with the Hospital Compliance Officer to draft Office of Inspector General (OIG) letters based on the Root Cause Analysis.
14. Supports and analyzes the annual Patient Safety Survey and collaborates with departmental leaders to address survey findings.
0. Handles product recalls and defective products through an established process and reports to FDA and/or the manufacturer as required under the Safe Medical Device Act.
1. Regularly updates the organization on TJC Sentinel Alerts, patient safety initiatives, and best practices for patient safety.
2. Supports effective responses to adverse occurrences and Sentinel Events, providing a proactive approach to reducing risk events.
3. Integrates patient safety as a priority into new processes and the redesign of existing processes, functions and services.
4. Participates in Failure Mode and Effects Analysis.
5. Oversees the creation, review, and refinements of the scope of the Patient Safety Program within the facility on an annual basis which includes ongoing assessment of climate of patient safety in the organization.
6. Coordinates the activity of the patient safety committee or function within the organization
7. Coordinates communication between stakeholders in the organization from staff to Board members related to patient safety activities and initiatives
8. Oversees the management and use of medical errors information. Reviews internal error data and information from external best practice organization and evaluates use of technology in driving initiatives
9. Recommends and facilitates change in the organization to improve patient safety based on identified risks
10. Leads initiatives to develop proactive risk assessment strategies across services to identify and mitigate patient safety risks
11. Develops a mechanism for internal communication of patient safety issues within a non-punitive work environment
12. Serves as an expert to departments in developing reliable delivery of care methodologies that incorporate best practice patient safety methodologies.
13. Supports the development of a recognition program aimed at improving patient safety
14. Establishes processes for patient and family engagement in the patient safety systems of the organization
15. Develops and manages budget related to patient safety activities in the organization
16. Ensures that data for decision making for patient safety is robust in terms of rigor and representative of the systems being examined.
17. Maintains confidentiality of patient information and the privileged status of information

19. Protects and respects the integrity of the Patient Safety Office in all decision-making processes and activities.
POSITION QUALIFICATIONS:
Required:
· Must have Bachelor’s Degree in patient care related field
· At least 3 years of experience (five years preferred) with progressive responsibility working in a quality/performance improvement and/or patient safety department
· Experience in leading meetings
· Experience leading interdisciplinary initiatives in process improvement or directly improving the reliability of healthcare delivery at the point of care.
· Experience in coordinating root cause analysis and intense analysis
· Strong skills in data interpretation
· Superior interpersonal skills
· Strong leadership qualities (task completion, motivation)
· Ability to handle multiple simultaneous tasks; strong skills in prioritizing and meeting deadlines
· Excellent communication skills
· Ability to work under pressure
Preferred:
· J.D, or Master’s in Nursing, Health Services, or Patient Safety
· Certified Professional Healthcare Risk Management (CPHRM)
