Position Summary:
The Health Coach is responsible for providing individualized, evidence-based lifestyle management coaching to participants. This role supports individuals in identifying health risks, developing personalized wellness plans, and achieving sustainable behavior change that promotes overall well-being.

Responsibilities:

• Provide personalized and holistic health coaching services focused on lifestyle management, including nutrition, physical activity, stress reduction, sleep, tobacco cessation and other behavior modification strategies.
• Conduct comprehensive assessments to identify health risks, readiness to change, and areas for improvement.
• Develop and implement individualized wellness plans that align with participant goals and organizational program objectives.
• Support participants through goal setting, accountability, and ongoing education to facilitate meaningful and lasting behavior change.
• Maintain accurate and detailed documentation of participant interactions, progress, and wellness plans to ensure continuity of care and program effectiveness.
• Stay current with research, trends, and best practices related to health coaching, and preventive health.
• Contact and engage eligible participants to promote enrollment and encourage ongoing participation in wellness initiatives.
• Demonstrate effective time management by conducting coaching sessions as scheduled and within established time limits.
• Collaborate with team members and leadership to support overall wellness program goals and client outcomes.
• Perform other related duties as assigned.

Requirements

To be considered for the Health Coach position, you should have:

• Health Coaching certification: Required
• 6+ months of experience: Required
• Skilled in utilizing technology for data management, communication, and client support
• Bachelor's Degree (4-year college or technical school): Preferred
• Other wellness certifications: Preferred
• Worksite wellness & health coaching experience: Preferred
