Purpose

This position will support the Northend locations at: Ballard Clinic, three HHOT sites Wintonia, Westlake and Morrison. This position is grant funded.

The provider will work a total of 0.6 FTE split between our Ballard Clinic and our Housing Health Outreach Team (HHOT) permanent supportive housing sites, one located in downtown Seattle. At the Ballard Clinic the provider will function as solo prescriber working in an interdisciplinary team (RN, MA, Behavioral Health, Psych, SW, Clinic Coordinator). This clinic follows a low-barrier access to care model accommodating both scheduled visits and walk-in same day visits. At our HHOT sites, the provider works in partnership with the onsite outreach RN and case management from the permanent supportive housing building partner. HHOT visits are often joint visits with the RN and may include team (RN and provider) outreach visits to patients’ apartments or common areas in the building. 

We have Obstetrics and Medical rounding available with our MD/DO positions.

Compensation:

  • The target wage range for the MD/DO position is $98.21 per hour to $121.50 per hour
  • The target wage range for the APP position is $63.53 per hour to $78.61 per hour  
  • Final offers are individually based on various factors, including skill set, years of experience, location, qualifications, work schedule and other job-related reasons. 
  • $3,000 sign-on bonus

Primary Responsibilities:

Direct Clinical Work

  • Provide patient centered preventative care for adults, as well as diagnoses and treatment of acute and chronic conditions in collaboration with interdisciplinary team (nurse, behavioral health, social work).
  • Use the principles of trauma informed care and harm reduction to inform patient care plans. 
  • Identify the need for referrals and provide overall care coordination in a team-based setting.  
  • Work with individuals of varying ethnicities, socio-economic levels, cultural, sexual orientation, and gender expression.  
  • Note:  Physician Assistant only – Will practice under a Practice or Collaboration Agreement with an MD/DO, based on clinical hours completed, per WA Medical Commission regulations. 

Indirect Clinical Work

  • Assure accurate and timely completion of all clinical records 
  • Completion of paperwork in a team-based approach. 
  • Complete necessary paperwork for patient care, such as prior authorizations and communications with outside specialists 
  • Timely response to communications and test results  
  • Provide feedback to appropriate teams and/or individuals when necessary
  • Perform other duties as assigned

Required Skills:

  • Ability to work in a high pressure, time-sensitive, and complex health care environment 
  • Ability to effectively work with individuals of varying ethnicities, socio-economic levels, varying languages, cultures and sexual orientations 
  • Ability to work with patients that have substance or mental disorders 
  • Ability to treat opioid use with medication assisted treatments 
  • Ability to meet and comply with HIPAA/Confidentiality policies and procedures 
  • Experience working with interpreters in a medical setting 
  • Demonstrates the ability to conduct discussions of a sensitive nature with staff and patients 
  • Ability to build relationships while maintaining appropriate boundaries 
  • Ability to communicate effectively in person, email and on the phone 
  • Ability to develop collaborative working relationships that foster a positive working environment with an emphasis on teamwork 
  • Attention to detail and ability to organize and manage time and tasks independently 
  • Ability to prioritize work assignments and adjust tasks as needed 
  • Proficient with email and electronic health records

Education/Experience Requirements:

  • Graduation from an accredited School of Medicine or Osteopathy, Nurse Practitioner or Physician Assistant Program  
  • Completion of Residency (for MD/DO/NP)  
  • Licensed as a Physician MD/DO, NP or PA-C with Washington State Department of Health
  • Board Certified  
  • Current BLS Certification  
  • DEA License issued in the State of Washington 

Preferred Requirements:

  • Preference will be given to providers with at least 2 years of clinical practice experience in community health, FQHC  
  • Experience with poly-substance use disorders 
  • Completion of Nurse Practitioner Residency in FQHC setting  
  • Experience with complex wound care, gender affirming care, PrEP, hepatitis C treatment, and people experiencing homelessness
  • Experience with motivational interviewing 
  • Contraceptive Implant Certification 
  • Bilingual skills  
  • Experience prescribing buprenorphine or other treatments for opioid use disorders, including past completion of a buprenorphine waiver certification course or specific continuing education to satisfy the requirements for the DEA's Medication Access and Training Expansion (MATE) Act.
