Responsible for program development and operational accountabilities for obstetrical patients requiring complex fetal care, as well as educating, and supporting family needs of patients seen and referred within Geisinger. Partners with Clinical Director(s), to have strategic oversight into programmatic development, function, and patient care rendered by the Center for Prenatal Pediatrics. Drives connectivity with community-based services, assuring access to complex fetal and pediatric care, and monitoring overall patient outcomes. Accountable for evaluating impact of the Center on patient satisfaction, while working to strengthen market share, avoiding leakage outside of Geisinger. Participate with designated leadership in review and revision of policies, procedures and structure and content of the program.Job Duties:
- Develop and oversee implementation of a system wide approach to complex fetal, newborn, and palliative newborn care.
- Research best in class approaches across the nation and incorporate standards of care and best clinical practices into program.
- Identify key performance indicators that reflect care of pregnant individuals with serious and/or complex prenatal fetal diagnosis, and/or life-limiting fetal illnesses.
- Monitor KPIs and modify strategy based on outcomes.
- In conjunction with the Institutes, Organizations, Hospitals, and Referral centers, identify opportunities for care models that support high quality care for pregnant patients, with complex fetal conditions.
- Continue to foster enhancement in functional perinatal care partnership with designated counterpart at Children's Hospital of Philadelphia.
- Assure program compliance with appropriate regulatory bodies.
- Develop, implement, and revise ongoing performance improvement activities; report such service activities at appropriate organization meetings, apply critical and analytical thinking and ongoing evaluation to the Performance Improvement process.
- Develop and coordinate comprehensive staff training programs related to vital conversations.
- Partner with appropriate community agencies to increase community awareness about advancing illness, patient wishes, goals of care to increase patient, family, and community engagement in a comprehensive approach to complex fetal care.
- Work proactively with payers and the clinical delivery teams to ensure coordinated hand-offs and smooth transitions when helping patients and families make transitions to palliative, transitional, and hospice care.
- Serve as liaison across programs, including referral sources, primary care providers, specialty and hospital services, payers, home care agencies, discharge planners and community groups and resources.
- Collaborate with senior leadership team to monitor program impact and budget adherence.
- Create business plans as indicated.
- Represent the program on committees as applicable.
- Continuously monitor the effectiveness of the program, develop evaluation process, including a subjective evaluation instrument to survey others on program effectiveness.

Position Details:

- Education: Bachelor’s Degree in Social Work or Healthcare Related Field required.
- This role is required to work on-sight at department location.
