About The Role
The Director of Home Health and Hospice provides leadership and oversight for all operations, growth, and development within Home Options, including both Home Health and Hospice services. This position drives service excellence, operational performance, and regulatory compliance while fostering a culture rooted in compassion, collaboration, and innovation.
In partnership with senior leadership, this role plays a key part in shaping care delivery, business strategy, and community partnerships to ensure our patients and families receive the highest quality care at every stage of life.
Qualifications
- Bachelor’s degree in healthcare, leadership, or business required; Master’s degree preferred.
- Current Montana RN license (or compact equivalent) required.
- Minimum of five (5) years’ leadership experience, preferably within home health, hospice, or related healthcare services.
- Proven experience in strategic planning, financial oversight, and service line development.
- Strong ability to build collaborative relationships and communicate effectively with providers, staff, and community partners.
- Demonstrated expertise in performance improvement, compliance, and patient-centered care delivery.
Key Responsibilities
- Provides strategic and operational leadership for Home Health and Hospice services, ensuring alignment with Logan Health’s mission, vision, and values.
- Develops and implements strategic goals, business plans, and quality initiatives that promote growth, patient satisfaction, and regulatory compliance.
- Oversees financial management, staffing, and productivity to meet established targets and ensure long-term program sustainability.
- Builds and maintains strong relationships with internal stakeholders, referring providers, and community partners to enhance service integration and continuity of care.
- Ensures compliance with all federal, state, and local regulations, including Medicare Conditions of Participation and other accreditation standards.
- Champions a culture of excellence, engagement, and accountability through team development, mentorship, and collaborative leadership.

Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
