Patient Care Coordinator Position Summary

Supports the overall business plan and strategic direction of the organization by ensuring that infusion services and resupply orders are received, qualified, and processed in a timely, efficient, accurate, and courteous manner; and collaborating with other functions to deliver whole patient care and world class service.

Patient Care Coordinator Essential Functions

  • Models and holds others accountable to the Barnes culture and acts as a coach and mentor for others in the organization.
  • Assesses the needs of the patient/partner and acts accordingly to provide exceptional service and whole patient care as efficiently as possible.
  • Collects information from customers, physicians, and referral sources to facilitate accurate order entry and evaluates eligibility based on payer guidelines.
  • Coordinates as needed with Transitional Care Coordinators, sales, and infusion pharmacy team members for appropriate clinical documentation to ensure a smooth transition of care.
  • Processes patient information accurately as required in software system (CMB, CPR, etc.)
  • Monitors recurring contacts list for patient re-orders.
  • Addresses PT concerns or alerts.
  • Responds to customer inquiries regarding supplies, shipments, products, and deliveries; and handles complaints.
  • Submits insurance verification, interprets coverage benefits, and collects payments.
  • Collaborates with other functions to facilitate whole patient care and meet partner needs.
  • Other duties as assigned.

Patient Care Coordinator Minimum qualifications and skills

  • High school diploma or equivalent required.
  • A minimum of two years of related experience is strongly preferred.

Working Conditions

  • This is a full-time, in-office position based in a professional office setting
  • Sitting for 8+ hours per day
  • Ability to lift up to 15lbs.
