The Clinical Support Nurse is primarily responsible for the day-to-day coordination of care for Hospice clients, supporting the Clinical Manager(s). Completes workflow and tasks within the EMR and outside of the EMR, assuring quality and compliance standards are met.  Manages incoming phone calls for triage support.  Responsible for analyzing data integrity and consistency of documentation and assessment processes.

  • Position: Clinical Support Nurse (RN) for Hospice
  • RN Salary: $85,000 - $90,000 annually, depending on experience
  • Service Area: Twin Cities Metro area. Plus, mileage reimbursement of .70 cents per mile.

Schedule: Full -Time, 1.0 FTE, weekend rotation: every other, 2 holiday rotations per year and on-call as needed

Option 1: Monday – Friday, 8:00am-5:00pm OR

Option 2: Four (4) weekdays, 10-hour days

Position Functions & Responsibilities:

  • Supports the clinical team with care coordination by identifying and responding to patient needs via triage calls.  Assists to monitor care delivery of clients and with care planning, visit scheduling, and phone call visits as needed.
  • Ensures proper maintenance of clinical records in compliance with local, state, and federal laws.
  • Completes continuing education programs to meet education and training needs of organization.
  • Completes assessments and audits ensuring completeness and compliance with federal and state regulations and organization policy
  • Reviews discharge and start of care documentation and coordinates with clinicians to make timely corrections as needed.  Utilizes back office for reviewing data and notifies clinical leadership of any problematic trends. 
  • Participates in quality improvement activities as assigned
  • Assists with chart audits as assigned
  • Ensures all orders and services received via an outside source are included in the client’s plan of care
  • Obtain verbal orders for clients and documents as applicable.
  • Acts as a clinical resource to personnel
  • Runs all applicable reports and completes workflow taking appropriate actions with high level of focus and accuracy. 
  • Acts as field RN when assigned by a clinical leader and assumes responsibility to coordinate client care for assigned caseload.
  • Promote self-growth and collegial relationships with others in the hospice and healthcare industry
  • Ability to come into the corporate office upon adequate notice from the manager.  There may be times when physical presence in the office are deemed necessary and will be responsible for being physically in the office when clinical managers are on PTO to cover as needed.
  • Other duties as assigned.
  • Ability to work every other weekend

Qualifications:

Experience

  • Minimum 2 years of hospice experience required.
  • Minimum of 3 years of experience working with people in the second half of life.
  • Experience in customer service/direct client care
  • Working knowledge of HCHB and ICD-10 coding
