The listed minimum pay reflects compensation for a full-time equivalent (1.0 FTE) position. Actual compensation may differ depending on assigned hours and position status (e.g., part-time).

Manager Care Mgmt Social Work :Manages the daily operations and supervision of the Care Management program. Provides expertise to Care Managers, Social Workers, and other appropriate hospital personnel regarding clinical management of specific patient populations, coordination of discharge planning and process improvement processes in accordance with state, federal and Joint Commission requirements. Provides education to physicians and other appropriate departments regarding care management. Assists Director of Care Management to assure departmental performance achieves the objectives of Capital Health as well as department specific indicators/targets for quality, employee satisfaction, customer satisfaction and financial targets.
- Provides supervision of staff to assure best practices in both social work and care management services. Assigns staff in accordance to skills, customers needs and regulatory requirements.
- Collaborates routinely with care management, hospital staff, managers, and physicians to assure that patient social work needs are provided effectively and efficiently.
- Continually reviews regulatory requirements for social work and discharge planning services to assure appropriate and mandatory services are provided.
- Develops annual performance measures and reviews with employee on an ongoing basis. Provides clinical supervision to LSW staff.
- Routinely reviews and analyzes staff performance and clinical outcomes via chart review, weekly length of stay meetings, daily rounds, and patient and staff satisfaction information.
- Exercises discretion and judgement to monitor educational plan and makes changes according to needs of staff and patients. Coordinates community discharge programs that help to reduce length of stay and resource utilization.
- Participates actively in system wide committees that impact on patient care, resource utilization, and program development. Attends system level meetings and represents care management in order to promote and develop good working relationships.
- Utilizes knowledge gained and ongoing updates of social work, care management, discharge planning, and health care services and systems to improve and expand patient care services and programs.
- Supervises RNs and care manager social workers and support staff to include hiring, conducting of performance evaluations, scheduling, and disciplinary actions.
- Collaborates with director to prepare the annual budget and to diagnose budget overages as evidenced by explanatory reports and documentation.
- Performs other duties as assigned.
