A Brief Overview

This role will provide a unique interdisciplinary health coaching opportunity at a state of the art Clinic for Health and Wellness located at the Sherwin-Williams Corporate Headquarters (SW) in downtown Cleveland. In addition to providing direct traditional Primary Care services, the clinic will include intentional integration of lifestyle medicine modalities to treat and proactively prevent disease. This role will work very closely with a team that is passionate about whole person health and well-being.

What You Will Do
 

  • Conduct individual and/or group coaching sessions following the health coaching model via in-person outpatient, telehealth, and EHR messaging modalities.
  • Facilitate the exploration of personalized mission reflection, personal health inventory creation, and action planning processes with patients, clients, and groups.
  • Provide individual and group education and evidence-based coaching interventions related to lifestyle behaviors, health management, and health promotion for purposes of improving physical and mental health.
  • Administer health behavior assessments and co-develop plans for health behavioral changes that increase quality of life and reduce risk of chronic disease diagnosis or progression.
  • Perform coaching sessions with compassion, empathy, and cultural competence to meet patients where they are, help them develop relevant health action planning steps, and track progress on goals.
  • Utilize positive inquiry, motivational interviewing, reflective listening, and other coaching strategies to elicit realistic and applicable lifestyle goals and action steps.
  • Connect patients with primary and specialty providers as applicable through coordination with care management and patient navigation services in order to maintain appropriate scope of practice, enable best practices for treatment, and connect to community/social services to meet patient needs pertaining to social determinants of health.
  • Develop shared goals and action steps related to skills, habits, and health behaviors of interest.Collaborate in interdisciplinary team setting to optimize patient experience and care outcomes.Conduct group sessions providing education and interventions related to well-being topics including but not limited to exercise, mindfulness, stress management, nutrition, resilience, and behavior change.
  • Support and work closely with coaches throughout the UH system to evaluate and improve coaching practices and programs.
  • Maintain accurate and detailed documentation in the electronic health record to maintain continuity between sessions and the care team.

  • For specific duties and responsibilities, refer to documentation provided by the department during orientation.
  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
 

Qualifications

 

Education

  • Associate's Degree in healthcare, health promotion, fitness, nutrition, culinary, nursing, counseling, or other related field or 2+ years of work experience listed below (Required)
  • Bachelor's Degree (Preferred)
  • Master's Degree (Preferred)

Work Experience

  • 2+ years healthcare, health promotion, or related experience (Preferred)

Licenses And Certifications

  • National Board Certified Health & Wellness Coach (NBC-HWC) (Required Upon Hire)
  • The selected candidate will be required to complete and maintain the American College of Lifestyle Medicine Board Certification within eighteen months of hire.
