The Columbia Presbyterian Community is seeking a highly skilled and compassionate Case Manager to join our team to create a warm hospitality experience for all our residents, staff and visitors. In this position, you will enjoy working with a passionate team to provide quality service throughout our community.

What You'Ll Do:

• Conduct initial assessments of residents' needs and develop individualized care plans
• Coordinate with healthcare providers to ensure residents receive appropriate medical care
• Monitor residents' progress and adjust care plans as needed
• Communicate regularly with residents and their families to provide updates on care plans and address any concerns
• Collaborate with interdisciplinary team members to ensure the highest quality of care for residents
• Maintain accurate and up-to-date documentation of all case management activities

What We'Re Looking For:

• Bachelor's degree in social work, nursing, or related field
• Minimum of 2 years of experience in case management, preferably in a skilled nursing or long-term care setting
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of CMS and DHEC regulations 
• Proficient in Microsoft Office and electronic medical records systems
• CPR Certification
• Knowledge of Electronic Medical Record Documentation

Minimum Qualifications and Skills are listed below:

• Bachelor’s degree in related field
• 1-2 of Long Term Care /Senior Living/CCRC Experience
• CPR Certification
• Knowledge of Electronic Medical Record Documentation

Prefer: 3 years’ Short term rehabilitation experience or previous case management experience
