The Columbia Presbyterian Community is seeking a highly skilled and compassionate Case Manager to join our team to create a warm hospitality experience for all our residents, staff and visitors. In this position, you will enjoy working with a passionate team to provide quality service throughout our community.
What You'Ll Do:
- Conduct initial assessments of residents' needs and develop individualized care plans
- Coordinate with healthcare providers to ensure residents receive appropriate medical care
- Monitor residents' progress and adjust care plans as needed
- Communicate regularly with residents and their families to provide updates on care plans and address any concerns
- Collaborate with interdisciplinary team members to ensure the highest quality of care for residents
- Maintain accurate and up-to-date documentation of all case management activities
What We'Re Looking For:
- Bachelor's degree in social work, nursing, or related field
- Minimum of 2 years of experience in case management, preferably in a skilled nursing or long-term care setting
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of CMS and DHEC regulations
- Proficient in Microsoft Office and electronic medical records systems
- CPR Certification
- Knowledge of Electronic Medical Record Documentation

Minimum Qualifications and Skills are listed below:
- Bachelor’s degree in related field
- 1-2 of Long Term Care /Senior Living/CCRC Experience
- CPR Certification
- Knowledge of Electronic Medical Record Documentation
Prefer: 3 years’ Short term rehabilitation experience or previous case management experience
