About This Role
The Clinical Services Director (CSD) provides strategic leadership and oversight of clinical operations within Seen Health’s PACE program. Reporting directly to the executive team (with a dotted-line to the Medical Director for clinical skills oversight), the CSD ensures delivery of high-quality, participant-centered care and compliance with PACE regulations. This role directs nursing services, clinic workflows, and care coordination while fostering interdisciplinary collaboration. The CSD is a unifying leader who sets the tone for consistent standards, strong participant outcomes, and an integrated team approach aligned with the Seen Health PACE-is-a-team-sport philosophy.Responsibilities
Leadership & Oversight
- Act as the Nursing Supervisor, with responsibility over home care, enrollment RN, RN care management, and clinic management RN. Oversee nursing and clinical staff hiring, performance, training, and competency development (RNs, LVNs, CNAs, PCAs, MAs).
- Serve as a member of the SGV leadership team, supporting census growth, policy development, and operational excellence.
- Drive accountability around smooth clinic operations, ensuring all roles contribute to on-time, high quality urgent/scheduled visits, specialist clinics, and telehealth services
- Champion interdisciplinary collaboration, bridging departmental silos and reinforcing PACE philosophy of integrated care.
Clinical Operations
- Ensure effective management of comprehensive case management for participants across settings: hospitalization, SNF, home health, and transitions of care.
- Oversee population health and wellness campaigns spanning both participants and staff (TB, vaccines, etc.).
- Oversee medication management, including on-site emergency/routine supply, ordering, and pharmacy coordination.
- Lead on-call/24-7 call structure: scheduling, documentation, escalation, and provider handoffs.
- Supervise intake and enrollment nursing functions to ensure timely assessments, care plans, and census growth goals.
- Oversee RN care plan completion, home care program operations, and change-of-condition care planning.
- Provide oversight of scheduling and coordination of contracted specialists (dentist, optometrist, podiatry, radiology, etc.) to ensure accountability to quality outcomes.
- Develop and manage telehealth protocols to expand participant access to care.
Quality, Compliance & Safety
- Co-develop and oversee infection control plan and compliance with federal/state PACE regulations.
- Ensure readiness for audits across nursing and clinical functions, including documentation, assessments, care plans, and on-call services.
- Participate in and lead Quality Improvement (QI) initiatives; identify trends, conduct root cause analyses, and implement corrective actions.
- Ensure compliance with CLIA waiver standards and lab utilization, including training and oversight of staff conducting in-house tests.
- Ensure seamless integration of palliative care and end-of-life services into the interdisciplinary team model.
- Promote safe, compliant clinic operations and a culture of continuous improvement.

Licensure/Certification
- Bachelor of Science in Nursing (BSN) required
- Master’s degree in healthcare management or related field preferred
- Professional certification in nursing leadership or geriatrics preferred
- Minimum 5 years’ experience with frail/elderly populations in acute, long-term care, or community-based settings
- Minimum 3 years of progressive management experience required
- Experience in home care or long-term care preferred
- Managed care and paperless documentation experience preferred
- Active State RN license
- BLS certification
- Valid driver’s license and proof of insurance
- Bachelor of Science in Nursing (BSN) required
