The Referral Specialist is responsible for the functions of the Connecting NJ System. This individual oversees the performance of the system, identifies and addresses problems, and performs a triage role for clients who seek services.
Essential Functions:
Contribute to The Cooperative’s commitment to equity and continuous quality improvement that aligns with the agency’s mission, goals, and advancement. * Effectively cross-collaborate with agency staff, leaders, and board members to foster a comprehensive approach of information sharing, working committees, and collective representation. * Routinely disseminate relevant information, presentations, training, and learning opportunities for internal program and agency staff. * When applicable, will participate in relevant staff development opportunities (trainings, summits, conferences, etc.) to increase skill development, elevate program goals, and engage staff members with timely key learnings. * Participates in coordination of Connecting NJ/Hub meetings which might include orienting new home visiting agencies to CNJ Link, participating in discussion and agreement among members about business rules to govern triage of referrals, and coordinating cross-training and shared in-service among Connecting NJ partnership. * Manages all referrals received, including Perinatal Risk Assessments (PRA), (faxed) one page referral forms and telephonic referrals; reviews and forwards triaged referrals to HV providers and reassigns to alternate programs as appropriate; communicates with prenatal providers and HV agencies about status of referrals with FHI. * Identifies resources for families and communicates with these families, providing information and referrals, and documenting contacts. * Provides feedback to Prenatal Providers and Home Visiting agencies, conducts reviews of referrals received and sent, participates in Continuous Quality Improvement (CQI) activities, and completes quarterly reports for the program funder. * Performs other duties as assigned.
Requirements
Knowledge, Skills, and Abilities to Perform Essential Functions:
- Associate’s degree in social work, human services, or nursing required; Bachelor’s degree preferred
- 1 year experience in case management, home visiting, or MCH health promotion programs
- 1 year experience working with vulnerable maternal and child health populations preferred
- Ability to develop rapport with clients from a variety of cultural and ethnic backgrounds
- Knowledge of community resources and sensitivity to community issues
- Basic computer skills necessary
- Excellent organizational skills; ability to prioritize work requirements
- Bilingual English/Spanish preferred
- Valid driver’s license, car, and current insurance policy
- Strong oral and written communication skills
Physical Requirements:
- Must be able to see to read documents and hear well enough to communicate with others
- Must be able to write, type, and use a telephone
- Must be able to operate a motor vehicle
- Must be able to lift up to 20 pounds
