Position Summary
The Advanced Practice Registered Nurse (APRN) provides expert hospice care within the framework of our mission and philosophy. The APRN evaluates, manages, and supports patients with advanced illnesses, collaborating closely with the Interdisciplinary Team (IDT) and Hospice Medical Director. This role includes conducting required face-to-face (F2F) encounters for hospice recertification, ensuring compliance with federal and state hospice regulations.
Compensation: $62.50
Essential Functions
- Apply advanced clinical judgment in the assessment, diagnosis, planning, and management of complex health conditions related to life-limiting illnesses.
- Provide holistic care guided by ethical principles, professional standards, and the ANA Code of Ethics for Nurses.
- Collaborate with the IDT to deliver coordinated, compassionate, and patient-centered care.
- Promote culturally and spiritually competent care, respecting diversity in culture, faith, gender, socioeconomic status, and sexual orientation.
- Educate patients, families, healthcare professionals, and community members on hospice and palliative care principles.
- Incorporate evidence-based research and best practices into patient care.
- Complete all documentation of F2F visits in the EMR within 24 hours of each encounter.
- Communicate promptly with the hospice team to ensure timely patient care coordination.

Ability to manage multiple tasks, prioritize effectively, and maintain orderly records and systems. • Strong verbal and written communication skills are essential for interacting with colleagues, clients, and stakeholders. • Must possess digital literacy, including familiarity with various computer applications and internet-based tools. Proficiency in using standard Microsoft/office software and applications (e.g., email, word processing, spreadsheets) is essential along with basic data entry skills, as well as the ability to quickly learn and adapt to new software and systems used in our office.

Requirements
Education And Experience:
- Master’s Degree in Nursing from an accredited school of nursing
License/Certification:
- AANP or ANCC board certification
- License has no encumbrance
- Current license in the state of employment.
- Valid driver’s license
- Current automobile insurance
- Reliable vehicle
