We’re currently seeking a detail-oriented and experienced Registered Nurse (RN) to step into the role of MDS Coordinator. This is an excellent opportunity to lead essential assessment processes and work closely with a supportive interdisciplinary team.
Position Overview
The MDS Coordinator is responsible for overseeing and managing the Minimum Data Set (MDS) assessment process. This includes evaluating resident care needs, coordinating care plans, and ensuring compliance with all federal and state guidelines related to documentation and reimbursement.
Key Responsibilities
- Participate in the pre-admission process to gather critical clinical data for MDS planning
- Set and manage ARD (Assessment Reference Dates) in collaboration with the care team
- Complete accurate and timely MDS assessments per CMS guidelines and facility protocols
- Conduct interviews and observations, review clinical records, and ensure precise MDS coding
- Maintain an organized MDS schedule and track assessment timelines
- Collaborate with department leaders to ensure documentation aligns with care delivered
- Perform other duties as needed to support the overall success of the clinical team
- Current and active Registered Nurse (RN) license in the state of Oregon

- Familiarity with the RAI process and case management practices
- Excellent communication and interpersonal skills
- High level of attention to detail and strong organizational skills
- Prior experience in MDS coordination or long-term care documentation is a plus
