Job Summary: The CNA Supervisor provides direct, compassionate care to residents in a long-term care setting while serving as a mentor and resource to the CNA team. This role ensures the consistent delivery of high-quality, person-centered care by assisting with daily resident needs, supporting team workflow, and fostering a safe, respectful, and dignified environment. The CNA Supervisor collaborates with nursing staff and leadership to coordinate resident care, maintain regulatory compliance, and promote a positive workplace culture.

- Provide direct care to residents, including assistance with activities of daily living (ADLs), mobility, dining, and personal care.
- Observe, document, and promptly report changes in residents’ condition to nursing staff.
- Serve as a role model by consistently demonstrating professional, compassionate care practices.
- Mentor and support CNAs through onboarding, training, and ongoing skill development.
- Provide guidance during shifts, ensuring team members follow care plans, schedules, and safety protocols.
- Communicate effectively with wellness nurses and leadership regarding staffing, workflow, and resident needs.
- Assist in organizing daily assignments, prioritizing resident needs, and ensuring appropriate coverage.
- Monitor staff compliance with infection control, safety, and resident care policies.
- Support documentation requirements by ensuring accurate and timely completion of records.
- Participate in quality improvement initiatives, staff meetings, and training programs.
- Build positive relationships with residents and families, addressing concerns with empathy and professionalism.
- Assist in planning and supporting engagement activities that enhance residents’ quality of life.
- Promote resident engagement and socialization through encouragement of daily routines and activities.
- Model professional standards, including proper uniform, visible name badge, and adherence to no personal device use while on duty.
- Support a multi-disciplinary approach to resident care, maintaining open communication with the care team regarding changes in diet, mobility, or emotional well-being.
- Demonstrate transparency in reporting all incidents affecting resident care, following the chain of command.
- Identify, address, and report inappropriate staff behavior in accordance with progressive discipline policies, ensuring all team members are held accountable to established standards.
- Comply with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all Plymouth Place policies and procedures.
- Maintain regular and punctual attendance at work and meetings.
- Attends in-service and education programs.

Requirements
Job Requirements:
- High school diploma or equivalent required.
- Current certification as a Certified Nursing Assistant (CNA) in good standing with the state.
- Minimum of 2 years of CNA experience in long-term care, assisted living, or related healthcare setting required.
- Prior experience in a leadership, mentoring, or supervisory role strongly preferred.
- Current CPR and First Aid certification.
Knowledge, Skills And Abilities Required:
- Understanding of long-term care regulations, infection control, and resident rights.
- Familiarity with care plans, documentation standards, and quality improvement processes.
- Knowledge of person-centered care practices and approaches to dementia and memory care.
- Strong leadership, mentoring, and coaching abilities.
- Effective communication and active listening with staff, residents, and families.
- Strong organizational and time-management skills to prioritize resident and team needs.
- Proficiency in basic computer systems and electronic health record (EHR) documentation.
- Skilled in problem-solving and conflict resolution within the care team.
- Ability to lead and motivate a team while holding staff accountable to standards.
- Ability to provide compassionate, dignified, and individualized resident care.
- Ability to adapt to changing resident needs and facility priorities.
- Ability to remain calm and effective in emergencies or high-pressure situations.
- Ability to collaborate effectively with a multi-disciplinary care team.
- Proficient with Microsoft Office Suite or related software.
- Solid organizational skills including attention to detail and multi-tasking skills.
- Ability to read, write and speak English sufficient to perform job duties and to interact and communicate with residents, families, and employees.
ENVIRONMENTAL AND PHYSICAL ACTIVITY REQUIREMENTS: The physical activities of this position constantly require effective talking, hearing, sitting, walking, and standing functions. The position also often requires exerting up to 50 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Must be able to perform activities with close visual acuity, such as reading a computer screen and reading print material. The functions of this position are conducted in a temperature-controlled environment.
OSHA Exposure Category: Category I – Position includes tasks that involve exposure to blood, body fluids, tissues; would be required to perform Category I tasks.
INTELLECTUAL/EMOTIONAL REQUIREMENTS: The occupant of this position must be able to perform the responsibilities of the position well under pressure. The position requires adaptability to performing
a variety of duties, often changing from one task to another without loss of efficiency or composure. This position requires the ability to deal with and find resolutions to problems and maintain a productive working relationship with employees and may be interrupted frequently to meet the needs and requests of others. It also requires adaptability to fluctuate between priorities daily while being responsive to needs as they arise.
