In this position, you will assess consumers applying for, or currently receiving, long-term care services to develop, coordinate, and arrange a variety of services to allow older adults and people with disabilities to get the continuity of care they need. Bilingual Requirement (English/Arabic)

  • This position is bilingual and requires duties to be carried out in both English and Arabic.  
  • The job offer will be contingent upon passing test(s) evaluating your proficiency in the bilingual requirements of the position.  
  • This position may receive a 5% differential pay based on passing the required language test.  

Summary Of Duties

As a Case Manager, you will:

  • Determine Long-Term Care Program Eligibility.
  • Develop detailed, individualized, and person-centered case planning for consumers.
  • Provide case management by coordinating the provision of long-term care services to consumers according to rules, regulations, and the individual case plans.
  • Work closely and collaboratively with the consumer, other agencies, medical and care providers, and community partners to provide benefits, services, and information.
  • Process applications, inquiries, and data according to policy timeframes, ensure appropriate documentation and forms are completed and sent, and communicate eligibility decisions to ensure delivery of benefits and services.
  • Assure that service delivery is provided in a culturally competent and trauma-informed way; assure that printed materials are available in preferred languages/formats; bilingual services available and facilities are accessible for all consumers.

Minimum Qualifications

A valid driver’s license and acceptable driving record are required for this position.

Bilingual in English and Arabic (the job offer will be contingent upon passing test(s) evaluating your proficiency in the bilingual requirements of the position).   

A bachelor's degree in a Behavioral Science, Social Science, or a closely related field

Or

A bachelor's degree in any field AND one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing)

Or

An associate degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing)

Or

Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).

  • Experience working with an equitable and trauma-informed approach. 
  • Experience building respectful and professional relationships with coworkers, supervisors, community groups and the public.
  • Experience problem-solving involving human services barriers with multiple teams ensuring all voices are heard. 
  • Experience using time management, organization, and prioritization skills to track and complete tasks.
  • Experience using time management and prioritizing caseloads with multiple competing deadlines.
  • Bilingual in English/Arabic is required.

Working Conditions

  • Fast paced environment with constantly evolving policies and procedures.
  • Travel is required to meet business needs and to attend employee training classes.
  • May drive a variety of different types of vehicles to remote places in a variety of terrain and possibly inclement weather.
  • Will come in contact with animals.
  • May be exposed to communicable diseases, such as the flu virus, Covid-19, and MRSA.
  • You will interact with a wide range of people including some who have experienced trauma and may exhibit challenges in controlling their emotions. 
  • You may encounter stressful situations that require quick decision-making to ensure safety of yourself and others.
