Position Overview:

We’re urgently seeking a Disability Support Worker to provide dedicated mental health experience in Liverpool & Campbelltown area. This role involves assisting clients in building life skills, accessing community services, and achieving their personal goals.

Key Responsibilities:

• Develop and implement individualised support plans tailored to client needs
• Assist with daily living skills, community engagement, and employment/education support
• Provide mental health support, guiding clients toward self-sufficiency
• Support clients with social activities, transport, and personal care as required
• Maintain accurate documentation and case notes

What We’Re Looking For:

• Qualifications: Cert III/IV in Mental Health or related field (or relevant experience)
• Experience: Background in Mental health or Disability
