A great opportunity has arisen to join the Digital Care Team as a Senior Independence and Wellbeing Practitioner - Digital Discharge working at West Suffolk House within Adult Social Care.

Your role and responsibilities 

You will work as part of the Digital Care Team to support patients, families and staff to use the Cassius Service particularly as part of the discharge pathways. Working closely with social workers, and other Independence and Wellbeing Practitioners and staff from across the locality to get the best outcomes for people as well as health colleagues both within the hospital and community settings.

This role will give you great hands-on experience of care technology in practice and delivering change. This work is rewarding and fast-paced.

Specific Tasks Include:

  • Supporting other professionals to consider care technology as part of discharge planning.
  • Support care technology assessment.
  • Supporting community referrals to prevent hospital admissions, linking in with Integrated Neighbourhood Team Coordinators.
  • Managing the referral process (including benefits tracking and recording).
  • Supporting communication and awareness raising of Cassius.
  • Managing decisions about eligibility.
  • Support the Alcove Team in prioritising installations.
  • Delivering training to therapy teams – both hospital and community based.
  • Shape and develop the role in line with business needs and changes.
  • Working as part of an integrated team with NHS, Social Care and Cassius.

You Will Need

  • Excellent communication skills.
  • The enthusiasm to contribute to a multi-agency approach for responding within urgent timescales. 
  • Resilience under pressure and the ability to remain positive when challenged. 
  • Ability to adapt to changes within the role as and when required.
  • Excellent knowledge of care technology and Cassius and how this can positively improve quality of life and maintaining adults’ independence.

This team seeks to improve the quality of life and independence of the people we work with by providing care technology at the earliest opportunity in an adult’s journey.  Within the digital care team, we continue to develop changes to improve the service to ensure we are providing care technology to those most in need. In particular with these roles, we will focus on minimising a person’s stay in hospital whilst maximising the opportunities our teams have to work with people in a community setting focusing on reablement and improving their health and long-term well-being.
